Chronic Care Staffing is Hiring a Remote Care Coordinator (CMA)! Location: Remote (Work From Home) Pay Range: $17.00 – $20.00 per hour Employment Type: Full-Time Employer: Chronic Care Staffing Overview Chronic Care Staffing (CCS) is seeking a dedicated Remote Care Coordinator (CMA) to join our growing chronic care management team. In this role, you will support patients managing chronic conditions by providing care coordination, health coaching, and remote patient monitoring services. This is a fully remote opportunity for a certified medical assistant who enjoys patient interaction, collaborative care, and helping individuals stay engaged in their long-term health goals. Key Responsibilities - Conduct monthly Chronic Care Management (CCM) outreach calls and patient enrollments - Perform health risk assessments and support care planning for chronic condition management - Assist with Remote Patient Monitoring (RPM) and transitional care follow-up - Communicate effectively with physicians, care teams, patients, and family members - Support medication adherence, symptom tracking, and self-management education - Collaborate with internal staff to ensure coordinated, patient-centered care - Connect patients with appropriate community and support resources - Maintain accurate documentation and meet required call volume standards in EHR systems - Ensure all interactions align with CCS guidelines and quality expectations
Job Requirements
- Required Qualifications
- Active, unencumbered CMA or RMA certification
- Current Basic Life Support (BLS) certification
- High proficiency in EHR systems and virtual documentation workflows
- Recent experience in a clinical or CCM care coordination setting
- Strong ability to work independently in a remote environment
- Excellent verbal and written communication skills
- Strong clinical reasoning, problem-solving, and critical thinking abilities
- Ability to organize and manage workload effectively in a distraction-free home office
- Preferred (Nice to Have) Qualifications
- Experience in care coordination, chronic care management, transitional care, or remote monitoring
- Familiarity with community resource navigation and patient support services
- Experience providing education on chronic conditions and medication adherence
- Background in quality measures, reporting, or CMS compliance
- Demonstrated ability to thrive in a fast-paced remote healthcare environment
- Additional coursework or certifications related to care or case management

Role Description The Customer Success Lead focuses on both the strategic management of the account (20%) and concentrates on the tactical operations (80%) by working with the operations team to provide optimal services to the account. The CSL must be able to work effectively and efficiently with both the internal support team and external clients. The CSL acts as the liaison and primary contact for the operations team and escalates any issues to the Customer Success Manager (CSM) as necessary. Assist the CSM with building client relationships within the client enterprise including business development, identifying additional service opportunities and continuous process improvement.
