Role Description We are seeking a compassionate and knowledgeable Stella Health Navigator to join our team. The ideal candidate will play a crucial role in assisting our members with their health needs, understanding their health benefits, and connecting them to partner solutions. This position involves: - Engaging with members to understand their health needs and provide personalized support. - Educating members on their health benefits and how to maximize them. - Connecting members with partner solutions and resources to address their health concerns. - Scheduling and coordinating doctor appointments and follow-up care. - Supporting members by connecting them with clinical education and other resources. - Serving as a liaison between members and healthcare providers to ensure seamless communication. - Assisting members in navigating the healthcare system and overcoming barriers to care. - Maintaining accurate and confidential member records and documentation. - Continuous documentation and evaluation of operational processes for improvement. Qualifications - Degree from applicable program of training and a minimum of 3 years clinical experience in an acute care setting required. - Active Registered Nursing license or equivalent within the state of practice or states in which Case Management is performed. - Bachelor's degree preferred. - Certification in related field preferred. - Experience in care navigation, case management, or a related field is preferred. - Strong understanding of health benefits and insurance plans. - Excellent communication and interpersonal skills. - Experience in scheduling and coordinating healthcare services. - Compassionate and patient-centered approach to care. - Ability to communicate effectively in English and Spanish preferred. • Answer high volume of inbound calls from and/or generate high volume of outbound calls to delinquent borrowers. • Resolve delinquent accounts by demanding and collecting the full amount due, establish a solid plan with the borrower to bring the loan current, or proceed with hardship assistance as appropriate. • Counsel borrowers on options for meeting their payment obligations, explain terms, and arrange repayment plans based on department procedures. • Discuss financial status, review monthly income and expenses, and determine reason for non-payment. • Offer Loss Mitigation assistance, and if appropriate, explain options and processes in detail. • Demonstrate accurate knowledge and strict application of federal regulations and collection laws (FDCPA). • Follow all company and department policies and procedures. • Utilize proper protocols, for borrowers, clients, state, and federal regulations, as well as best practices. • Maintain and document mortgage collection systems and databases with all activities and communications. • Process mortgage loan payments in accordance with established guidelines. • Consistently meet or exceed monthly goals. • Successfully complete annual regulatory compliance training. • Additional duties as assigned.
