• Provide 1:1 virtual physiotherapy consultations to patients enrolled in our 12-week Better Health Programs • Support patients in developing and maintaining safe, accessible, and effective physical activity routines tailored to their needs and goals • Educate and motivate patients around movement, injury prevention, posture, and body mechanics • Collaborate with a multidisciplinary team, including physicians, dietitians, and counsellors • Participate in training to become confident in delivering care using our lifestyle medicine framework
Job Requirements
- Registered Physiotherapist licensed to practice in Ontario
- Training and/or experience in lifestyle medicine, exercise prescription, behavioural change, or motivational interviewing
- Training/experience with chronic conditions – Type 2 Diabetes and Metabolic Syndrome
- Strong communication and interpersonal skills
- Comfortable with technology and delivering virtual care
- Able to work independently and within a collaborative team setting

• Counsel clients or patients individually and/or within family sessions, to assist in overcoming dependencies, adjusting to life, or making changes. • Make internal referrals to IOP if little to no improvement occurs within 8 weeks of OP therapy. • Guide clients in the development of skills or strategies for dealing with their problems. • Develop and implement treatment plans based on clinical experience and knowledge, establishing SMART goals with the patient. • Treatment plans are to be developed 72 hours after intake and updated / modified every 4 weeks. • Fill out and maintain client-related paperwork, including client diagnostic records and progress notes. • Progress notes are to be developed 48 hours after every session. • Prepare and maintain all required treatment records and reports. • Evaluate clients' physical or mental condition, based on review of client information. • Collaborate with other staff members to perform clinical assessments or develop treatment plans within 2 days of establishing care with the existing patient. • Refer patients, clients, or family members to community resources or to specialists as necessary. • Learn about new developments in counseling by reading professional literature, attending courses and seminars, or establishing and maintaining contact with other social service agencies.
