Schedule: Monday-Friday, 9a-5p Location: Boston, MA Position Overview We are seeking an experienced and compassionate Registered Nurse Case Manager to join our team in a fully remote capacity. This role offers flexibility, with the option for candidates to work a hybrid schedule if they prefer to spend a few days on-site at our Braintree, MA location—though on-site work is not required. The RN Case Manager will play a critical role in coordinating care, supporting patients, and ensuring high-quality outcomes across assigned cases. Key Responsibilities - Manage and coordinate patient care plans across the continuum of care - Conduct clinical assessments and ongoing case reviews - Collaborate with interdisciplinary teams, providers, and stakeholders - Advocate for patients by identifying needs, resources, and appropriate interventions - Ensure compliance with clinical guidelines, policies, and regulatory requirements - Maintain thorough and timely documentation within case management systems Qualifications - Active Registered Nurse (RN) license - Prior experience in case management or care coordination preferred - Strong clinical judgment and organizational skills - Ability to work independently in a remote environment - Excellent communication and collaboration skills Work Environment - In person role conducting telehealth assessments.
