Nurse Practitioner, Virtual Care Management *Must hold an active Florida NP license About PreemptiveAI PreemptiveAI is on a mission to redefine virtual care through our innovative remote patient monitoring and care management platform, Preemptive Clinic. The Role As a Nurse Practitioner on our Virtual Care Management team, you'll deliver a member-first clinical experience across virtual visits, care planning, and care team support. You'll report to the Clinical Director and partner closely with our physician medical directors, care managers, and care navigators. This is a clinical role with a strong operational component. You'll provide direct patient care, guide the care team on day-to-day clinical decisions, and help build the workflows and processes that make our care model work. Responsibilities - Conduct virtual onboarding E/M visits, including history, assessment, documentation, orders/referrals, and follow-up planning - Provide clinical decision-making between visits; ensure timely care and appropriate escalation - Coordinate with member PCPs and specialists to support continuity of care - Review RPM data, alerts, and trends to identify members needing outreach or escalation; document clinical interpretation and route follow-ups appropriately - Escalate clinical concerns to the Clinical Director and/or medical directors per protocol - Partner with care managers to design, review, and approve individualized care plans aligned to evidence-based guidance and member goals - Provide day-to-day clinical guidance to care navigators and care managers; unblock escalations and mentor on documentation quality and escalation judgment - Help refine clinical workflows, templates, and protocols to improve safety, efficiency, and member experience - Identify gaps and propose improvements based on recurring needs, outcomes, and team feedback Requirements - Active, unrestricted Florida NP license in good standing - 2+ years of NP experience providing direct patient care (virtual care experience is a plus) - Strong E/M fundamentals: assessment, treatment planning, documentation, ordering/referrals - Comfort managing chronic and/or complex conditions; sound judgment on when higher-acuity evaluation is needed - Experience with telehealth, RPM, care management, or care navigation preferred - Excellent communication skills with a balance of empathy, clarity, and follow-through - Strong collaboration skills and good escalation judgment within a clinical team - High attention to detail across documentation and follow-up for multiple members - Comfortable in EHRs and digital tools; learns new systems quickly - Leadership or mentorship experience with clinical teams is a plus - Builder mindset: excited to improve workflows and playbooks as the organization grows - Ownership mentality and comfort operating in an early-stage environment where priorities evolve Compensation & Benefits We offer a competitive salary and benefits commensurate with experience, with the opportunity to make a meaningful impact in a mission-driven, fast-growing company.
