Role Description We're looking for a Registered Nurse who brings strong clinical judgment, calm communication, and a genuine passion for caring for their patients. As a Nurse Care Manager, you'll support Medicare-age members virtually through: - Proactive check-ins - Remote patient monitoring (RPM) - Chronic care management (CCM) - Review, triage, care coordination, and education You'll work alongside our Nurse Practitioners, physician leaders, and care team to deliver a concierge-level experience that helps patients stay healthy, independent, and ahead of problems. This is a fully remote position with consistent weekday hours (Monday–Friday, ET). Nurse Practitioner, Virtual Care Management *Must hold an active Florida NP license About PreemptiveAI PreemptiveAI is on a mission to redefine virtual care through our innovative remote patient monitoring and care management platform, Preemptive Clinic. We combine modern technology with high-touch clinical support to help members stay ahead of health issues, reduce avoidable hospitalizations, and make healthcare easier to navigate. The Role As a Nurse Practitioner on our Virtual Care Management team, you'll deliver a member-first clinical experience across virtual visits, care planning, and care team support. You'll report to the Clinical Director and partner closely with our physician medical directors, care managers, and care navigators. This is a clinical role with a strong operational component. You'll provide direct patient care, guide the care team on day-to-day clinical decisions, and help build the workflows and processes that make our care model work.
