RN Case Manager
Libertana
• Responsible for the clinical care and coordination of services provided to patients in their home. • Providing clinical services, such as comprehensive assessments and management of medications. • Organizing and coordinating services needed by recipients. • Documenting patient interventions and responses. • Collaborating with the Social Worker on the Case Management Team. • Completing assessments and reassessments based on enrollment timelines. • Managing complaints from clients, families, or friends. • Ensuring compliance with Federal, State, Local, and HIPAA Regulations.
Job Requirements
- Must have a current, unsuspended, un-revoked license to practice as a Registered Nurse in California.
- Knowledge of the current Nursing Practice Act.
- Current CPR certification.
- Proficient in the use of computers.
- Minimum two (2) years experience in a home health organization, primary care clinic, or a health facility.
- One (1) year experience in healthcare quality management preferred.
- Current and satisfactory report on pre-employment physical examination including TB Screening Test or chest X-ray as required by Agency policies and procedures.
- Excellent judgment and knowledge of current nursing practices.
- Good written and verbal communication skills.
- Fluent in English.
- Must be a licensed driver with an insured automobile in good working order.

Role Description We are currently seeking a o9 Consulting Manager to join our team in the United States (US). Candidates must currently be a legal resident of the United States. As an Integrated Demand and Supply Planning (IDSP) o9 Consulting Manager, you will help us implement o9 Solutions to improve operating performance for our clients. NTT DATA’s IDSP consultants work closely with clients on initiatives to balance demand and supply by helping them design their demand and supply planning processes, overlay enabling technology, and ultimately, implement the executive Integrated Demand and Supply Planning process.
