Job Summary and Responsibilities The Virtually Integrated Care Coordinator role is crucial for assisting nurses in patient care coordination and supporting daily operational workflows within the innovative Virtually Integrated Care Program (VIC). This position involves actively engaging with admitted patients to educate them on the VIC program and its benefits. The coordinator facilitates safe and timely patient discharge by coordinating follow-up appointments, supporting medication coordination workflows, and facilitating communication among care teams, the pharmacy, and patients.
