The Opportunity: The Population Health Support Nurse (USRN - NY) is responsible for monitoring and improving health outcomes for defined patient populations by integrating clinical expertise with health principles aimed at reducing the need for acute healthcare. The nurse will focus on disease management, preventative care, care coordination, and addressing social determinants of health. The nurse will work collaboratively with interdisciplinary teams to ensure seamless transitions of care and promote patient self-management. This position is remote and offers you the flexibility of working from home. Essential Duties and Responsibilities: - Conducts telephonic visits with clients to complete assessments, review recent hospital or ER visits, provide disease management education, and address flagged client needs. - Responsible for meeting productivity standards set by the organization. - Works with other members of the team to meet KPI’s and program targets. - Utilizes critical thinking skills to ensure client needs are prioritized. - Support program enrollment and monitor participation. - Support clients who are post-discharge from hospitalization, emergency department, or skilled nursing facilities. - Communicate findings as needed to the clients’ primary care physician and/or specialists. - Document communication with clients, caregivers, and providers timely and accurately. - Connect clients to care and support: (Not limited to): - Behavioral Health - Condition Management to Benefits Engagement - Provider Referrals - Educate client, caregiver, and family as appropriate and provide information on available community resources. - Review client needs/referrals as identified by caregivers and others - ensure appropriate follow-up occurs. - Seek clarification from caregiver and client as needed to formulate clinical judgement for next best actions. - Document accordingly in the EHR and other software no later than 24 hours after client contact. Required Skills/Abilities: - Experience working with at-risk and elderly populations. - Ability to work independently and as part of a broader care team. - Demonstrate flexibility and adaptability in managing changing priorities and patient needs. - Ability to foster a positive work environment and collaborate effectively with team members. - Proactive in resolving problems. - Proficient with Microsoft Office Suite. - Strong problem-solving skills. - Ability to leverage community resources to benefit clients. Education and Experience: - US Registered Nurse - NY License - Three (3)+ years of experience working in hospital or home care preferred - Strong preference for experience with elder population in a home health care, hospice/palliative or facility setting. A caregiver who excels in this role will: - Work directly with management of assigned facilities/units to review and discuss consistent staffing levels that reflect the appropriate number of licensed and unlicensed staff to meet quality and safe patient staffing and operational needs. - Proactively generate and review 24-hour staffing projections. - Seek coverage via phone calls and/or texts for shifts and special circumstances. - Conduct Lack of Work process according to Standard Operating Procedure when necessary to maintain safe and productive staffing. - Utilize staffing and scheduling knowledge to identify and resolve complex staffing issues that directly affect patient care. - Utilize multiple computer systems to communicate with nursing leadership of all hospitals. - Work collaboratively with nurse leadership to review current and anticipated staffing needs. - Work collaboratively with nursing institute timekeeping to ensure proper coding of caregiver’s time. - Facilitate scheduling of PRN caregivers and agency caregivers to maintain operations for the assigned units. - Complete appropriate documentation utilizing Kronos for caregiver schedule changes. - Be a point of contact for unit call offs. - Accurately record, document and report call offs according to SOP. - Compile ad hoc and scheduled reports for Chief Nursing Offers, Nursing Directors and Nurse Managers. - Attend and successfully complete required training. Minimum qualifications for the ideal future caregiver include: - High School Diploma/GED and 2 years of customer service experience - OR Associate's Degree may substitute for up to 1 year of the stated experience - OR a Bachelor's Degree may substitute for up to 2 years of stated experience - Proficient in MS Office, Excel and database software Preferred qualifications for the ideal future caregiver include: - One year of staffing/scheduling experience Physical Requirements: - Requires manual dexterity for constant data entry and ability to sit for long periods of time. - Ability to use a telephone without TTY. Key Responsibilities - Assist with onboarding new clients, including maintaining records of FATCA/CRS classifications, GIIN registrations, and entity information. - Collect, organise, and review self-certifications, W-forms, and supporting documentation. - Input and maintain accurate data in internal AEOI systems, dashboards, and trackers. - Monitor reporting deadlines across jurisdictions and support the team in ensuring timely filings. - Liaise with clients to obtain outstanding information and respond to routine queries. - Support in preparing and reviewing XML submissions for FATCA and CRS filings. - Assist with the reconciliation of filing acknowledgements and follow-up on rejections or discrepancies. - Maintain proper filing of correspondence, records, and supporting documents in line with internal policies. - Support senior staff in preparing management reports, trackers, and internal updates. - Provide general administrative support to the AEOI team as required. - Ensure timely billing in accordance with our internal procedures and actively manage accounts receivables. - Attendance at training sessions provided by Walkers to ensure continued development and maintenance of AEOI knowledge.
