The Wellness and Recovery Specialist is responsible for care coordination and/or care management activities focused on the Wellness and Recovery of members. ***This position requires a valid, active, certified peer specialist. How you will make an impact: - Responsible for coordinating and managing care activities with a focus on the wellness and recovery of members. - Key responsibilities include identifying opportunities to engage members and their families in forming a supportive recovery network. - Developing and implementing onsite psychiatric discharge planning education at Recovery and Resiliency sites. The role collaborates with Stabilization Teams, advocating for members in discharge planning education, resolving barriers, and facilitating service transitions. - Serves as a resource for staff with decision-making and problem-solving. - Initiating and maintaining contact with assigned individuals and providers to assess members' responses to services. Minimum Requirements: - Requires an AA/AS and a minimum of 2 years of experience in health services or behavioral health, or a peer support services role, or a combination of education and experience providing an equivalent background. - Valid, active, certified peer specialist Preferred Skills, Capabilities, and Experiences: - Knowledge of care coordination and case management concepts is highly preferred. - A BA/BS is preferred.
