Role Description Dans le cadre d’un remplacement pour congé maternité, le CRT Ténarèze et Armagnac recrute un(e) Infirmier(e) Diplômé(e) d’État Coordinateur(trice) (IDEC). Missions principales: - Coordination et mise en œuvre du CRT: - Réaliser les visites à domicile lors de l’admission et décider de l’admission en collaboration avec le médecin coordonnateur et la direction. - Procéder à l’évaluation multidimensionnelle (sociale, médico-sociale, prévention santé). - Élaborer, coordonner et évaluer les plans d’accompagnement personnalisés. - Assurer le suivi de la file active (admissions / sorties). - Coordonner les interventions des professionnels à domicile (soins, accompagnement, activités). - Être le référent privilégié des bénéficiaires et de leur entourage. - Assurer la transmission des informations en cas d’hospitalisation. - Animation et gestion d’équipe: - Animer les réunions d’équipe du CRT. - Planifier les interventions et organiser les tournées. - Gérer les absences et assurer la continuité de service. - Gérer le planning des astreintes IDE de nuit. - Développer et évaluer les besoins en formation (élaboration du plan de formation). - Développement partenarial et travail en réseau: - Initier, entretenir et formaliser les partenariats (conventions). - Coordonner les relations avec les partenaires médico-sociaux. - Planifier les interventions d’experts (ergothérapeute, psychologue, diététicienne…). - Participer à l’animation du réseau territorial (CPTS, groupes de travail…). - Contribuer à l’élaboration d’outils de communication. - Gestion administrative et qualité: - Alimenter et suivre le système d’information partagé. - Suivre les indicateurs d’activité et tableaux de bord. - Participer à la démarche qualité et à la gestion des risques. - Veiller au respect des procédures internes et de la législation. - Rédiger bilans, rapports d’activité et assurer la traçabilité. Qualifications - Diplôme d’État d’Infirmier exigé. - Expérience confirmée en gériatrie. - Une expérience en dispositif de coordination serait appréciée. Requirements - Compétences techniques: - Connaissance du secteur sanitaire et médico-social. - Maîtrise du parcours de la personne âgée. - Capacité à piloter un projet. - Capacités d’analyse et de synthèse. - Maîtrise des outils informatiques (Word, Excel, outils numériques, visioconférence). - Élaboration d’indicateurs, bilans et tableaux de bord. - Compétences organisationnelles: - Sens de l’organisation. - Capacité à prioriser. - Réactivité. - Autonomie. - Compétences managériales et relationnelles: - Gestion et mobilisation d’équipe pluridisciplinaire. - Travail en réseau. - Aisance rédactionnelle et communication orale. - Diplomatie. - Capacité à créer une relation de confiance avec les usagers et leurs aidants. - Savoir-être: - Esprit d’initiative. - Dynamisme. - Créativité. - Éthique professionnelle.
