Chronic Care Management – Health Coach

Cardiovascular Associates of America - CVAUSA

• Develops a patient coaching relationship that supports the patient care plan and quality of life for the patient • It is a trusted and empathic support for patients and families dealing with chronic conditions • Communicating abnormal findings to the cardiology care team • Monitors and encourages patient adherence to care plans and participation • Effectively communicates findings to the care team • Supports new patients in the enrollment process • Works as a liaison for the patient with the practice and providers • Identifies and escalates changes to providers to reduce events such as hospitalizations or urgent care visits • Works effectively with the EMR and CCM platform • Basic monitor troubleshooting with patients for the CCM platform • Ability to follow clinical protocols. • Participate in continued team education through shared learnings. • Contribute to a high-quality service both to physicians and patients. • Stays current and complies with state and federal regulations/statutes and company policies that impact the employee's area of responsibility.

Job Requirements

  • High School diploma or GED
  • 1 year of experience in remote chronic care management
  • Experience with cardiology patients - specifically heart failure
  • Registered Nurse (RN), Medical Assistant (MA), or Certified Nurse Assistant (CNA)
  • Experience with EMRs – specifically Athena, NexGen, GEMMS and e-clinical works
  • Great communication and multi-tasking skills
