Job Summary: - This is a hybrid position with a mix of remote and in-person work across King County. The YMCA of Greater Seattle is a contracted partner to provide case management services across King County as part of HealthierHere's Community Hub. The Community Hub Navigator reports to the Community Hub Supervisor and provides resource navigation to support individuals in identifying and achieving their goals related to whole-person health. Functions of the Community Hub Navigator may include but are not limited to: following best practices in assessing and addressing Health-Related Social Needs (HRSN) of individuals; connecting clients to appropriate internal and external resources to address health and social needs; and building strong relationships with internal and external partners, including YMCA staff and clients. This position carries a caseload of clients, and the number of clients may vary based on assignments by the HealthierHere Community Hub. Client interactions are often virtual through phone calls, text messages, and emails. Some in-person training, meetings, and community outreach will also be part of this role. What you'll get from working at the Y - Membership to the YMCA of Greater Seattle - Medical, Dental, Vision, and Life Insurance - Retirement with generous employer contributions - Free access to mental health resources - Rapidly accruing paid time off (PTO) - Discounts on qualifying YMCA of Greater Seattle childcare, day camp, overnight camp and outdoor leadership programs 25% for part-time staff, 50% for full-time staff - Some benefits only available to full-time staff Hiring Range: $25.00 - $28.00/hour Responsibilities - Maintain and support a caseload of clients with complex social and health-related needs across King County - Provide timely outreach, intake, enrollment, and assessment to all assigned clients - Develop individualized goals and service plans to assist referred clients in connecting to internal and external resources related to social services (housing, employment, food access, etc.), healthy lifestyles, primary medical and behavioral health care, and other services as assigned - Based on knowledge of the Social Work field and profession, follow best practices for YMCA Healthy Living department approaches to addressing Health-Related Social Needs, including efficiently and successfully connecting clients to services in the community - Help clients navigate the social service and healthcare landscape in King County, including those with complex needs related to housing, substance use support, behavioral health, and more - Provide information and education to clients to help them become more confident in accessing resources and be an active participant in their care coordination - Ensure program outcome requirements are met and services are rendered appropriately by understanding contract/grant terms and conditions - Utilize best practice approaches in the field, including trauma-informed care, motivational interviewing, social service navigation, etc. - Maintain clear, professional, and timely participant contact notes, service plans, progress updates, outcomes, and other documentation in designated databases - Collaborate with other YMCA staff and/or community partners to plan and facilitate activities, including visits to partner organizations in the community - Builds and maintains collaborative relationships with funding liaisons, other service providers, and other community organizations as assigned - Complete required trainings as directed in established timeframes - Attend outreach events such as resource fairs and community gatherings to promote services available to individuals - Other duties as assigned WORK ENVIRONMENT This position will include a combination of office, remote, and field work.
