Description ● Serve as the primary point of contact for patient questions and concerns regarding their referrals, imaging, labwork, medications and other treatment plan needs. ● Collaborate with healthcare providers and facilities to ensure that all aspects of a patient’s care are synchronized. This involves managing referrals, coordinating tests and procedures, and ensuring timely delivery of care services. ● Coordinate and communicate patient medications and authorization information with pharmacies and insurance providers. ● Communicate with patients or healthcare facilities regarding the status of patient labs, imaging, and other medical services. ● Complete or receive schedule requests and schedule patients using appropriate resources and knowledge for all appointment types to fulfill treatment plans. ● Utilize critical thinking skills to identify and solve problems, evaluating options, and implementing solutions. ● Maintain accurate and up-to-date patient records, demographic information, as well as document all interactions, interventions, and outcomes to ensure continuity of care and compliance with regulatory standards. ● Monitor patient progress and satisfaction to identify any areas for improvement. ● Treat patients with empathy and respect while conducting oneself in a professional manner in written and verbal correspondence. ● Other duties as assigned.
