JOB DESCRIPTION: SUMMARY OF POSITION The Clinical Care Coordinator is responsible for managing a caseload of clients, documentation and charting in electronic health record, communicating with multidisciplinary teams, educating community on available programming, and ensuring adequate patient care, with a main focus on veterans, military and their families receiving quality outpatient counseling services. They also review recommendations from the patient's mental health care providers and coordinate treatment and community resources available to the patient through the Military Services program and partnered organizations. Military cultural competency and a well-developed understanding of community resources for outpatient counseling is crucial. The care coordinator should be engaging and energetic while maintaining professional relationships with the community they will serve and are capable of engaging the Veteran and civilian community using a variety of modalities including online, virtual, and in- person means. We are seeking candidates in Central Standard Time Zone or Mountain Standard Time Zone. Candidate must reside in: Texas, Arizona, Utah, Colorado, Kansas, Oklahoma, Nevada, New Mexico, Washington to be considered. ESSENTIAL DUTIES & RESPONSIBILITIES - HIPAA Compliance: Perform all duties within HIPAA regulations. - Policy Implementation: Implement policies and procedures across the enterprise. - Care Coordination: Engage and link referrals for outpatient counseling services with providers across the US and abroad using shared-decision making model to ensure patient care meets individualized needs and program contractual standards. - Patient Safety: Conducting follow-up calls to assess risk level for individuals who have indicated history of suicidal ideation, provide safety management as necessary, and deliver timely referrals for services - Credentialing: Contribute to the management of resource database and provider network credentialing - Admissions and Treatment Authorization: Authorize outpatient treatment services as per program specific protocols. - Communication: facilitate communication and collaboration among diverse community settings as well as across both internal and external teams involved in patient’s care. Demonstrate adaptability and active engagement as a team player - Outreach: Increase positive awareness of Centerstone Military Services in diverse community settings and the mission to empower veterans and their families to thrive; Connect with local and national organizations with complementary and similar resources to help shape specific Centerstone programs. - Engagement: Drive eligible patients and their families to utilize Centerstone's programs while also identifying and addressing barriers to engagement. - Program Development: Assess programs to determine efficacy and areas of improvement.
