Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. You’ll play a critical role in building trusted relationships to make people healthier, helping bring flexible, comprehensive and personalized health evaluations right to people’s front doors. Job highlights The visit, which lasts about 45 minutes, allows for time to connect one-on-one and answer health questions. It includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You’ll use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicine, order lab tests or alter people’s current treatment regimen. Role Description Le Secrétariat général des assemblées est, à titre principal, responsable du processus de validation et d'adoption des délibérations de la collectivité et rend exécutoires ses actes administratifs. Par sa position stratégique d'intermédiaire entre l'administration départementale, la direction générale et le Cabinet, il veille à la sécurisation des actes et dispose, par conséquent, d'une vision transversale de toutes les politiques publiques départementales. Celle-ci est renforcée par la consolidation du rapport d'activités des services relevant de ses missions de coordination. Il a en charge en outre la mise en œuvre des règles relatives au statut des élus départementaux. - 14 réunions du Bureau réunissant l’exécutif du Département. - 11 réunions de la commission permanente donnant lieu à l’adoption d’environ 850 délibérations. - 25 réunions de commissions internes pour les travaux de l’Assemblée départementale. - 5 réunions du Conseil départemental donnant lieu à l’adoption d’environ 100 délibérations par an.
