Role Description The Wraparound Facilitator implements the Wraparound process for youth and families identified and referred to the agency for the Safe at Home West Virginia program. The goal of Safe at Home WV is to safely reduce reliance on in-state and out-of-state congregate care for youth ages 12-17. Safe at Home WV will provide for trauma-focused treatments delivered in an environment that safely preserves family relationships and empowers families to help meet their own needs. The primary goals of Wraparound intervention are to support family strengths, promote community involvement and ownership, and provide the needed resources to families to improve family well-being and reduce the risk of child abuse and neglect. The Wraparound Facilitator will coordinate, create, and individualize services and supports to fit the unique needs of the child and family while building upon their strengths. Responsibilities - Consults with Program Manager regularly and as needed - Responsible for contract compliance and oversight of program policies and procedures - Participates in quality improvement activities - Ensures that documentation is thorough, accurate, and timely - Ensures that pertinent data is collected and complied as required - Obtains and maintains proficiency certification to administer CANS assessments - Builds rapport and cooperation/service coordination with families and other community social service providers - Obtains needed reports and documents as needed from external sources - Plans and facilitates Wraparound Team meetings of the youth, family, and service providers - Develops and implements individualized plans of care in collaboration with the Wraparound Team - Identifies and develops formal and informal services/resources available to the youth, family, and community - Conducts home/site visits with youth and/or family in accordance with the needs determined in the individualized Wraparound plan - Tracks and monitors youth and family progress and adapts the Wraparound plan as needed and documents all pertinent information in a timely fashion - Produces written reports as needed - Maintains client records in compliance with organizational goals and objectives, as well as external regulatory guidelines - Identifies service gaps and collaborates with community stakeholders and existing service providers to resolve them - Responds to client crises - Attends trainings as needed to ensure quality service provision and fidelity to wraparound model - Other duties as assigned Qualifications - Bachelor’s degree in Social Work, Counseling, Psychology, or a related field - Two years of post-college work experience providing direct services to children and families - Possess knowledge of mental illness diagnoses and behavioral disorders in children - Ability to collaborate closely with a team of family members and professionals to develop and implement individualized plans of care - Flexible with schedule in order to meet with youth and family wraparound team members - Bilingual skills are preferred (Spanish) Salary Range USD $40,000.00 - $41,000.00 / Year

• Lead 3–4 one-hour group therapy sessions per day using a structured, evidence-based curriculum. • Facilitate group discussions using a “plug and play” approach from approved modalities to meet members where they are within the guardrails of our clinical framework. • Collaborate with prescribers, care navigators, and recovery coaches. • Monitor member progress and document outcomes. • Support a positive, recovery-oriented environment for members. • Engage in local community outreach to promote Groups’ mission.

• Make 35–45 outbound sales calls daily to prospective students • Explain course details, fees, batch timings, and career benefits • Handle queries and objections related to pricing, courses, and placements • Convert inquiries into confirmed admissions • Maintain accurate call records and update lead status in the system • Process admission forms and student documentation • Follow up on pending fee installments • Generate fee receipts using invoicing software • Coordinate with the branch team on student-related matters

• Provide immediate support and guidance to individuals experiencing emotional distress or crisis, using telephonic and chat platforms • Deliver single-session counseling, in-the-moment support, and crisis management in a supportive, ethical, and professional manner while maintaining client confidentiality • Conduct thorough assessments to determine appropriateness for short-term counseling • Manage high-risk situations, including suicide, self-harm, child welfare, and substance use concerns, and working closely with individuals at risk • Collaboratively develop clear and concise treatment and safety plans with the caller • Refer the caller to appropriate internal or external resources as part of a holistic care approach • Maintain accurate and clear clinical notes in our case management system • Able to pivot while maintaining the highest levels of professionalism in ever changing and ambiguous situations
