Our care management team is seeking an RN Care Manager to provide personalized care and support to caregivers and their loved ones. No night shifts or weekends required, and no on-call duties! As an RN Care Manager, you'll assess needs, create individualized care plans, conduct home visits, and ensure exceptional consumer care. What You Will Do - Conduct home visits in assigned territory to assess home safety, gather changes in health status, and provide clinical coaching and support including review of medications - Following an established care plan, provide ongoing coaching and support to caregivers via app, including resource referral, and reinforcement of best practices. - Develop trusting relationships with caregivers to influence change and reduce caregiver burnout - Document and review records for accuracy and timeliness, ensuring compliance with regulatory and accreditation standards - Other projects and duties as assigned. - Perform other duties as assigned. What You Will Bring - Must hold an active Registered Nurse (RN) License in the state of employment - Minimum two years of recent experience in direct care of elders or adults with disabilities - Ability to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required - Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting - Proficiency in common software and web-based applications (e.g. MS Office, case management systems) - Additional state-specific qualifications may be required.
