Winners Circle Group of Texas provides Targeted Case Management and Mental Health Rehabilitation Services to adults and children within several locations across Texas. RESPONSIBILITIES: - Deliver virtual therapy to children, adolescents and young adults in foster care - Maintain a full caseload - Develop an individual service plan tailored to meet the clients individual service needs - Assess, diagnose and treat children, adolescents and young adults with behavioral and emotional disturbances using the DSM-5/ICD-10 criteria - Coordinate with treatment team members and the family in the provision of services to meet the goals developed in the individual service plan. - Provide appropriate documentation for service delivery including treatment goals, progress notes and billing and submit documentation as specified by organizational policies. - Conduct clinical assessments to determine diagnostic eligibility and treatment needs within the priority population as needed (if caseload is not at capacity) - Monitor client progress, assessing intervention effectiveness for those with severe and persistent mental illness - Ensure compliance with agency policies, regulatory guidelines, and complete reporting/training accurately and timely - Participate in monthly clinical meetings - Meet with the Clinical Director to review cases, share updates, and collaboratively address any challenges or concerns. - Conduct quarterly trainings as needed for nonclinical staff - Assist with development of mental health/community resource directory for each site.
