What You'll Do - Serve as the primary contact for patients via phone and secure messaging - Help patients understand healthcare processes, benefits, and next steps - Schedule appointments and coordinate follow-ups - Verify insurance eligibility and assist with referrals - Document interactions accurately in EMR/EHR systems - Assist with prior authorizations and prescription requests - Obtain and organize medical records and supporting documentation - Escalate complex concerns to appropriate departments - Maintain HIPAA compliance and protect patient confidentiality - Collaborate with internal teams to ensure seamless patient experiences Qualifications - High school diploma or equivalent required - 1+ year of experience in healthcare, call center, customer service, or administrative support preferred - Strong communication and problem-solving skills - Basic knowledge of medical terminology is helpful - Comfortable using computers, EMR systems, and Microsoft Office - Reliable internet connection and ability to work independently - Healthcare certification is a plus but not required. Role Description The Patient Outreach Representative is a critical role at P3 Health Partners that helps ensure our patients are educated and scheduled for the care they need. Qualifications - High school diploma, GED, or equivalent experience. - Proficiency with Microsoft Office applications, particularly Outlook, MS Teams, and Excel. - 1+ years of customer service experience. - Effective verbal and listening communication skills. - Ability to work on site 5 days a week 8 a.m. to 5 p.m. Pacific time.
