Care Coordinator

RiseWell Community Services

Role Description RiseWell Community Services is looking for a Full-Time Care Coordinator in our State Aid Program. This role offers remote work; however, it would require occasional work in the field (NYC & Long Island Regions) serving participants. - Provide care coordination and health promotion services to individuals throughout Long Island. - Assist individuals with severe mental illness and/or 2 or more chronic health conditions to avoid costly inpatient hospitalizations, emergency room visits, and unnecessary care. - Assist individuals in obtaining Food Stamp and Social Security benefits. - Oversee and coordinate access to all services (behavioral, medical health, and social service supports) to assist individuals in staying healthy. - Complete all required documentation according to standards and no later than 24 hours of service delivery. - Connect participants with behavioral health specialists, primary medical care, housing services, vocational support, and entitlements. - Ensure participants stay on top of appointments and receive the vital care needed for a healthy life. Qualifications - Minimum of a Bachelor’s degree with at least 2 years relevant experience serving children; OR - Registered Nurse with 2 years relevant experience serving children; OR - Master’s Degree with 1-year relevant experience serving children. - Clean, valid NYS Driver’s License. - Fingerprinting, criminal record check, and all necessary clearances from the Justice Center. Job Requirements

  • Minimum of a Bachelor’s degree with at least 2 years relevant experience serving children; OR
