Location: Remote (Must reside in MST or EST time zone) Employment Type: Full-Time Schedule: Monday–Friday Pay Rate: $28 – $32 per hour Overview Chronic Care Staffing is seeking an experienced and compassionate Remote Care Coordinator (RN) to join our chronic care management team. This fully remote position is ideal for a registered nurse who thrives in a structured telehealth environment and is passionate about improving outcomes for patients managing chronic conditions. In this role, you will provide care coordination, patient education, and remote monitoring support while serving as a key communication bridge between patients and providers. Your work will directly support quality care delivery and long-term patient success. Key Responsibilities - Conduct monthly Chronic Care Management (CCM) outreach calls and complete verbal enrollments - Perform Health Risk Assessments (HRAs) and support Transitional Care Management (TCM) services - Assist with Remote Patient Monitoring (RPM) follow-up and chronic condition tracking - Educate patients and families on diagnoses, medications, and treatment plans - Collaborate with providers and internal teams to ensure coordinated, patient-centered care - Promote adherence to care plans and encourage self-management - Document all patient interactions accurately in EHR systems - Maintain compliance with HIPAA, CMS, and CCS quality standards - Meet productivity and call volume expectations while delivering empathetic care
Job Requirements
- Must Have
- Active, unencumbered Registered Nurse (RN) license
- Must reside in MST or EST time zone
- Current BLS certification
- Recent experience in a clinical or CCM care coordination setting
- High proficiency in EHR systems
- Proficiency in Google Suite
- Strong communication, clinical reasoning, and problem-solving skills
- Excellent verbal and written communication abilities
- Ability to work independently in a HIPAA-compliant home office (private workspace, locking door, dual monitors, high-speed internet)
- Nice to Have
- Prior experience in care coordination, chronic care management, transitional care, or remote patient monitoring
- Familiarity with community resource navigation
- Experience providing patient and family education for chronic conditions
- Knowledge of quality measures, CMS compliance, or reporting
- Demonstrated success working in a remote team-based environment
- Bilingual in English and Spanish
