Humana is looking for a Long Term Care Functional Screen Specialist to join the Inclusa team, where you will support members in the Wisconsin Family Care program. Reporting to the Supervisor, Care Coaching, you will use the State Department of Health Services (DHS) screening tool to assess member needs and eligibility. Main responsibilities: - Schedule member appointments for annual or change-based functional eligibility re-determinations, optimizing time and travel resources. - Use the DHS screening tool to assess members' functional abilities, health, and required supports. - Collaborate with staff to ensure assessment data aligns with internal records. - Conduct in-home, face-to-face interviews and enter assessment data into the designated online system to determine level of care. - Submit documentation promptly according to the established guidelines. - Attend meetings and training to stay updated on processes, screening tools, and state directives. - Participate in quality assurance activities to maintain screening competence and reliability. - Maintain required certification through ongoing skills testing, per State of Wisconsin standards. Use your skills to make an impact Required Qualifications - Four-year bachelor's degree in a Health or Human Services related field with one (1) year of experience working with at least one of the Family Care target populations OR be a Wisconsin licensed Registered Nurse with one (1) year of experience working with at least one of the Family Care target populations. - Long Term Care Functional Screener (LTCFS) certification-Selected individual must meet requirements and complete online screener certification training course(s) and become certified as a functional screener. **The Family Care target group population is defined as: frail elders and adults with intellectual, developmental, or physical disabilities*** Preferred Qualifications - LTCFS Certified - Previous Family Care case management/screener experience Additional Information - Workstyle: Field - Work Location: Madison - Travel: up to 50% throughout Madison and surrounding areas. - Typical Workdays/Hours: Monday – Friday, 8:00 am – 4:30 pm CST Driving This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits. Mileage reimbursement is provided for work-related travel. Eligible mileage includes: - Travel from your home to your first work location of the day. - Travel between client or assignment locations during the workday. - Travel from your final work location back to your home. WAH Internet Statement To ensure Home or Hybrid Home/Office employees' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: - At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. - Satellite, cellular and microwave connection can be used only if approved by leadership. - Employees who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense. - Humana will provide Home or Hybrid Home/Office employees with telephone equipment appropriate to meet the business requirements for their position/job. - Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information. Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
