Complex Case Manager, RN

Highmark Health

• Assess health management needs of assigned member panel • Utilize data/analytics to identify clinical interventions • Manage an active case load of members in case management • Conduct outreach, develop care plans, encourage behavior changes • Help members coordinate care and address barriers

Job Requirements

  • 7 years of any combination of clinical, case management and/or disease/condition management experience, provider operations and / or health insurance experience
  • Bachelor's Degree in Nursing
  • Current State of PA RN licensure OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) or WV or DE or NY is required.
  • Advanced training and experience in cognitive behavioral therapy (CBT), motivational interviewing or dialectical behavior therapy (DBT)
  • Experience working with the healthcare needs of diverse populations
  • Understanding of the importance of cultural competency in addressing targeted populations

As a Care Manager you’ll join a passionate care team providing personalized care and support to caregivers and their loved ones. Your role will involve conducting home visits, assessing care needs, crafting individualized care plans, and ensuring exceptional care for caregivers and care recipients. What You Will Do - Provide ongoing coaching, support and guidance to caregivers, developing trust, building confidence and reinforcing best practices to influence change and reduce caregiver burnout. - Conduct home visits and use the Careforth app to support caregivers in real time, following care plans and evaluating needs and progress towards goals. - Document home visits as related to plan of care; review case record documentation for accuracy and timeliness, ensuring compliance with regulatory and accreditation standards - Other projects and duties as assigned.
