Role Description Trillium Health Resources has a career opening for a Support Transition and Reentry (STAR) Project Coordinator to join our STAR team! The STAR Project Coordinator is responsible for: - Collaboration with correctional facilities. - Point of contact for all justice system entities. - Receiving and reviewing STAR program referrals. - Acting as the liaison between STAR, DMH/DHHS, and all correctional facilities. - Coordinating FACT team referrals and providers. - Developing project plans and tracking FACT project details for effective program administration of FACT Teams. On a typical day, you might: - Provide support to the STAR Team to ensure members receive whole person care focused on members’ needs including physical health, behavioral health, and social determinants of health. - Maintain organized electronic and shared drive files for program materials and reports. - Assist with preparation and formatting of program-related correspondence, reports, presentations, and meeting materials. - Receive and log STAR and FACT program referrals into designated software platforms. - Schedule internal and external meetings with justice system partners and cross-functional teams. - Serve as a program point of contact for general inquiries and routing of information to appropriate staff. Qualifications - High School Diploma/GED and six (6) years of experience in human services, social services, healthcare, or business field; OR - Associate’s degree and four (4) years of experience in human services, social services, healthcare, or business field; OR - Bachelor’s degree and minimum two (2) years of experience in human services, social services, healthcare, or business field; OR - Equivalent combination of education/experience. - Must have a valid driver’s license. - Must reside within 75 miles of Trillium’s Catchment of North Carolina. - Must be able to travel within catchment as required. Requirements - One (1) year of experience working with criminal justice agencies. - One (1) year of experience managing and organizing business projects. - Education, training, and work experience that shows a history of learning and applying the skills needed to work with a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diverse backgrounds.
