The Registered Nurse 1, CareLine, is responsible for providing telephonic triage directional patient care advice for general and specific illnesses, health related issues, client counseling, patient advocacy, health education and referral and resource management to ChenMed patients and their families. Providing on-call coverage, the incumbent in this role provides remote clinical advice and assessments within license and as possible given technology and medium. The registered nurse collaborates with primary caregivers and others on the interdisciplinary care plan team to provide a team approach of care. The schedule for this position is as follows: Tues, Thurs, Fri 0530-0830 & Saturdays 0530-1430ESSENTIAL JOB DUTIES/RESPONSIBILITIES: - Connects with patients via phone or video call. Interviews and questions patients to collect health history and uses a computer system to record and store comprehensive and focused data relating to the health needs of patients and families.  - Provides health assessment and treatment solutions, monitors patient health and at-home care, aids in emergency scenarios and promotes patient wellness.  - Based on technology available, monitors a patientâs blood oxygen levels, heart rate, respirations, blood pressure and blood glucose as well as other assessment measures.  - With the help of video chatting, identifies patientâs symptoms and conditions. Analyzes data to determine the appropriate health maintenance and identify appropriate outcomes for patient and family.  - Collaborate with on-call providers as needed to support expected clinical outcomes for the patient and family.  - Evaluates and documents progress toward the anticipated outcome. Assist in ensuring achievement of optimal patient outcomes using Telemedicine. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. Position Summary The Member Care Coordinator position is a non-clinician role that works in collaboration with the Care Management staff and/or quality improvement staff to support the multi-disciplinary team approach of patient care by meeting key performance indicators (closing care gaps, reducing hospitalizations, readmissions, ED (Emergency Department) utilization, and PMPM costs) and other organizational mandates as designated. The Member Care Coordinator may work remotely within regions to cover the needs across the state and/or may work on site at CCPN (Community Care Physician Network) practices. Member Care Coordinators may directly assist members by increasing their ability to improve their health outcomes. They also help design and implement systems to ensure the smooth operation of office functions and to support the Care Team. Job Description Title: Triage Team Lead Location: Remote Employment Type: Full-time Exempt Position Overview: The Triage Team Lead is responsible for overseeing daily intake operations for FEDlogic’s employee-facing Triage Team. This role ensures that incoming consultation requests are reviewed timely, prioritized, documented, and routed to the appropriate Experts in compliance with FEDlogic standard processes. The Team Lead serves as the operational bridge between Services and Quality & Care Leadership; driving workflow efficiency, quality assurance, regulatory compliance, and continuous process improvement. This is a working-leader role: the Team Lead carries a case load while coaching staff, managing queues, escalating complex cases, and supporting system enhancements during FEDlogic’s CRM transformation (from Zendesk to Salesforce). Key Responsibilities: Operational Leadership - Supervise and support a team of Triage Specialists handling intake requests via phone, email, and ticket requests. - Monitor real-time queues, SLAs, turnaround times, and workload distribution. - Ensure timely assignment of cases to the correct Expert group (Medicaid, Medicare, SSI/SSDI, VA, RIB, CWC, etc.). - Serve as escalation point for complex triage matters, coordination of triage priorities, or urgent triage matters. - Partner with Workforce Management leadership on triage staff oversight and coverage schedules. Cross-Functional Partnership - Collaborate with Client Success on internal triage rules and communication protocols when needed. - Coordinate with Connecting with Care (CWC) on outreach workflows. - Escalate systemic issues to leadership with root-cause analysis and proposed solutions. Required Qualifications: - (For Internal Candidates only) 1 + year(s) experience in the Services Department performing triage ticket intake or consultations. - Strong knowledge of Medicare, Medicaid, SSI/SSDI, VA and employer-sponsored plans. - Experience managing queues and service level agreements (SLA) in Salesforce. - Experience writing standard operating procedures (SOP's) and desk guides - Strong documentation and analytical skills. - Ability to manage competing priorities in a high-volume environment.
