Role Description Assurer, en complémentarité et transparence avec la cheffe de délégation, l'interface terrain/DDTM ; vous vous répartirez la tâche pour contribuer à la présence de la DDTM sur le territoire. Porter les objectifs de transition écologique des territoires pour dépasser les contradictions apparentes. Identifier les questions sensibles, anticiper les conflits pour proposer et organiser des réunions de concertation, des notes... - Accompagnement des collectivités : les inciter à articuler leurs projets, à formuler une stratégie. - Contractualisation Etat/collectivités : accompagnement au Contrat de Relance et de Transition Ecologique. - Grands projets du centre Manche : copiloter le projet partenarial d'aménagement "entre 2 havres" dans une stratégie de recomposition littorale. - Autres projets liés à l'énergie nucléaire et éolien en mer. L'ensemble de ces missions nécessite une collaboration avec les services métiers de la DDTM, DREAL, ministère, conseil de l'Etat, agences... Affectation du poste : Coutances/Saint-Lô Qualifications - Connaissance générale des grandes thématiques de l'aménagement (env, aménagmt, mobilité, habitat, risques, planif...) - Maîtrise des règles de fonctionnement des collectivités territoriales et des services de l'État. - Présenter une expérience dans le domaine de l'aménagement du territoire. - Connaissance du territoire, des acteurs et des enjeux (à acquérir). Requirements - Capacité d'analyse, de synthèse et d'argumentation. - Formuler des propositions réalistes au vu des jeux et compétences d'acteurs et des financements éventuellement nécessaires. - Prise d'initiatives. - Savoir rédiger des notes, courriels. - Utilisation courante des outils informatique/bureautique. - Compétence cartographique souhaitable. Job Description Title: Triage Team Lead Location: Remote Employment Type: Full-time Exempt Position Overview: The Triage Team Lead is responsible for overseeing daily intake operations for FEDlogic’s employee-facing Triage Team. This role ensures that incoming consultation requests are reviewed timely, prioritized, documented, and routed to the appropriate Experts in compliance with FEDlogic standard processes. Key Responsibilities: Operational Leadership - Supervise and support a team of Triage Specialists handling intake requests via phone, email, and ticket requests. - Monitor real-time queues, SLAs, turnaround times, and workload distribution. - Ensure timely assignment of cases to the correct Expert group (Medicaid, Medicare, SSI/SSDI, VA, RIB, CWC, etc.). - Serve as escalation point for complex triage matters, coordination of triage priorities, or urgent triage matters. - Partner with Workforce Management leadership on triage staff oversight and coverage schedules. Quality & Compliance - Enforce documentation standards, intake completeness, and HIPAA compliance. - Collaborate with Quality & Care to review trends, error rates, and coaching needs for triage staff. - Work with Leadership to implement corrective action plans for recurring intake issues. - Maintain policy adherence across all workstreams. - Support leadership on all ad hoc triage needs. Coaching & Development - Conduct side-by-sides, call monitoring, and ticket reviews. - Deliver real-time coaching and formal performance feedback to triage staff. - Participate in hiring, onboarding, and readiness certification for new Triage staff. - Identify high-potential staff for advancement into Expert roles. - Lead team huddles and knowledge refreshers. Systems & Process Ownership - Serve as Triage SME for Zendesk/Salesforce workflows, intake forms, routing logic, macros, and automation. - Partner with leadership and IT during testing, UAT, and rollout of new workflows. - Document SOPs, decision trees, and escalation pathways for leadership. - Identify bottlenecks and recommend efficiency improvements. Cross-Functional Partnership - Collaborate with Client Success on internal triage rules and communication protocols when needed. - Coordinate with Connecting with Care (CWC) on outreach workflows. - Escalate systemic issues to leadership with root-cause analysis and proposed solutions. Required Qualifications: - (For Internal Candidates only) 1 + year(s) experience in the Services Department performing triage ticket intake or consultations. - Strong knowledge of Medicare, Medicaid, SSI/SSDI, VA and employer-sponsored plans. - Experience managing queues and service level agreements (SLA) in Salesforce. - Experience writing standard operating procedures (SOP's) and desk guides - Strong documentation and analytical skills. - Ability to manage competing priorities in a high-volume environment. The Role The Registered Nurse (RN) Team Lead will function as the Lead of a larger multidisciplinary treatment team. They will work closely with clients and monitor providers in order to provide the best care possible to our eating disorder clients. They will be responsible for assessing clients, contributing to the multidisciplinary treatment plan, providing weekly coaching to parents/loved ones, completing concurrent utilization review, and evaluating a client’s overall care in order to ensure they are progressing towards their treatment goals. They will also interface with clients by facilitating, and co-facilitating groups throughout the week. They will also serve as a liaison with clients, families, providers, community organizations and other health related service agencies to provide quality care to our clients. The RN specializes in working with clients with eating disorders, is thoughtful, sensitive, respectful, flexible, and brings a loving, positive attitude to our expert Clinical Team. Shift times will vary and may include on-call hours at night and on weekends. What You'll Do - Perform focused nursing assessments utilizing remote patient monitoring devices. - Oversee a team of therapists, dietitians, and care partners providing recommended treatments for clients. - Document all client interactions appropriately and within designated timeframes. - Conduct weekly family coaching calls. - Coordinate care and program services with patient, family, treatment team, and outpatient team. - Educate clients, and family members when appropriate, regarding medical complications of eating disorders. - Educate clients, and family members when appropriate, regarding psychiatric medications. - Provide in-the-moment feedback by utilizing between-session messaging to motivate and support clients and families. - Alert medical and clinical professionals to intervene during emergencies. - Maintain strict client confidentiality. - Participate in Staff meetings. - Participate in scheduled Partnership Meetings. - Participate in scheduled Family Partnership Meetings. - Facilitate or co-facilitate groups as appropriate. - Attend supervision and department meetings. - Participate in initial and ongoing training. - Provide case management for clients, ensuring effective communication with those involved in the recovery process, including school administrators, law enforcement, attorneys, etc. - Communicate with clients, family members, team members, & outpatient teams in a timely and consistent manner. - Give direction and direct feedback to different team members. - Other related duties as assigned based on need. Who You Are - An RN with an active, unrestricted, unsupervised RN license, with willingness to obtain additional RN licenses or Compact license in multiple states. - 3+ years experience as an RN in an eating disorder or behavioral health setting. - Minimum 1 year of supervisory experience. - Strong communicator, both verbally and in writing. - Ability to foster teamwork and create a cohesive work environment in a virtual setting. - Experience treating clients with eating disorders and Disordered Eating preferred. - Ability to demonstrate understanding of a variety of models and theories of eating disorders, trauma, mental illness, and related issues. - Knowledge of philosophies, practices, policies and outcomes of models of treatment, recovery, relapse prevention, and continuing care for dually diagnosed populations. - Understanding of diagnostic criteria for co-occurring conditions and ability to conceptualize modalities and placement criteria within the continuum of care. - Understanding of diverse cultures and gender specific issues and ability to incorporate needs of gender and culturally diverse groups into practice settings. - Excellent organizational and time management skills. - Ability to prioritize workload and work independently.
