Role Description This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants are ineligible to apply. The mission of the VISN 21 CRH Genomics Program is to provide comprehensive genetics care to Veterans with limited access to genetics care locally. This individual is responsible for developing the strategic and operational plan and executing on the clinical programs, ensuring that CRH services are completed at the highest standard. This CRH Genomics Section Chief is required to exercise a high level of leadership and organizational skill, independent judgment, problem-solving ability, and content expertise in program development, implementation, and evaluation. The position requires the ability to communicate effectively with field staff and VISN, CRH, and VA Central Office (VACO) leadership on a variety of clinical, policy, and administrative strategic priorities. Qualifications - Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. - United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. - Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. - Proficiency in spoken and written English. - Board certified or eligible in Internal medicine or family medicine. Requirements - Assist the CRH Specialty Service Chief with responsibilities as assigned for developing, organizing, and directing the VISN CRH Genomics Program and CRH Genomics staff. - Provide education on CRH actions and activities related to the CRH Genomics Program to various disciplines via multiple levels of training. - Ensure efficient operations of the VISN CRH Genomics Program are met and utilizes resources to ensure these operations meet the mission and goals of the VISN and national CRH Program. - Review incoming consults to determine whether patients would benefit from care through the CRH Genomics program. - Perform initial patient assessments including histories and physical examinations. - Provide care to Genomics Program Veterans, which includes coordination of care. - Order diagnostic studies as appropriate for Veterans receiving care within CRH Genomics Program. Role Description The VHA Nationally Designated-Clinical Resource Hub (CRH) is a strategic program designed to provide Team Based Primary Care, Specialty Mental Health and other specialty services in each VISN across the Enterprise, utilizing existing and emerging technologies. Tele health technology improves Veterans' access to health care by matching clinical provider supply with demand, especially in rural areas where it is difficult to recruit. The registered nurse provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes: - Systematic data gathering - Assessment - Appropriate nursing judgment - Evaluation of human responses to actual or potential health problems The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses). The registered nurse, as a licensed professional, will be responsible and accountable for his/her practice. Demonstrates proficiency in practice based on conscious, deliberate planning. Virtual RN services utilize care/case management principles to coordinate care and provide patient management and support using health informatics. Tele health technologies facilitate access to care and improve the health of Veterans with the specific intent of providing the right care, in the right place, at the right time. Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care. The Clinical Resource Hub (CRH) Registered Nurse supports the location to and from which care is delivered, including but not limited to VA clinical locations and provider alternate worksites. Supports the VISN CRH which may provide telehealth and face-to-face services not only to clinical areas within the VISN CRH but also other VISN CRHs throughout the nation. Provides direct care to one or multiple panels of patients. The physician serves as a primary care physician within the VISN 21 Clinical Resource Hub (CRH). The physician will provide virtual primary care services to VA clinics facing staffing shortages within VISN-21, which comprises northern and central California, Nevada, and the Pacific Islands including Guam. The physician will work with an inter-professional team of nurses, pharmacists, and mental health providers to deliver team-based primary care. The position is remote; however, we do require that the provider travel to the individual clinic they are serving to deliver in-person care every quarter through VA-funded travel. - Evaluate, diagnose, treat, and provide consultation for non-surgical adult patients of all ages, without life-threatening complications and with a wide variety of conditions. - Assess, stabilize, and determine the disposition of patients with emergent conditions consistent with medical staff policy regarding emergency and consultative call services. - Collaborate with/oversee mid-level providers such as nurse practitioners and work collaboratively with other specialties (and subspecialties) in and outside of the VA system to manage or co-manage patients' health. - Technical proficiency in using video conferencing platforms, proficiency with using an electronic medical record, and an aptitude to navigate systems-challenges that occur with the delivery of telehealth. - Written and verbal communication skills to function well within a virtual care team. - Additional duties as assigned. This position is part of the VISN Clinical Resource Hub (CRH) Virtual Psychotherapy Program for Caregivers (VPPC) Team, located in VISN 21. Staff within the CRH provide interprofessional services to VISN facilities that are underserved or experiencing gaps in clinicians. Conducts mental health assessments and treatment plans and psychotherapy to caregivers, families, couples, and groups to treat a variety of mental health diagnoses and disorders. Works closely with the Program of Comprehensive Assistance for Family Caregivers (PCAFC), the Program of General Caregiver Support Services (PGCSS), and other CSP staff to ensure a seamless transition to appropriate services for caregivers. Coordinates with the Substance Abuse Treatment Program, PTSD program, M2VA, and other Behavioral Health services as needed. Provides crisis intervention for caregivers in Behavioral Health in the outpatient setting. Performs other related duties as assigned.
