Role Description The VHA Nationally Designated-Clinical Resource Hub (CRH) is a strategic program designed to provide Team Based Primary Care, Specialty Mental Health and other specialty services in each VISN across the Enterprise, utilizing existing and emerging technologies. Tele health technology improves Veterans' access to health care by matching clinical provider supply with demand, especially in rural areas where it is difficult to recruit. The registered nurse provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes: - Systematic data gathering - Assessment - Appropriate nursing judgment - Evaluation of human responses to actual or potential health problems The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses). The registered nurse, as a licensed professional, will be responsible and accountable for his/her practice. Demonstrates proficiency in practice based on conscious, deliberate planning. Virtual RN services utilize care/case management principles to coordinate care and provide patient management and support using health informatics. Tele health technologies facilitate access to care and improve the health of Veterans with the specific intent of providing the right care, in the right place, at the right time. Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care. The Clinical Resource Hub (CRH) Registered Nurse supports the location to and from which care is delivered, including but not limited to VA clinical locations and provider alternate worksites. Supports the VISN CRH which may provide telehealth and face-to-face services not only to clinical areas within the VISN CRH but also other VISN CRHs throughout the nation. Provides direct care to one or multiple panels of patients.
