Role Description This is not a typical social work or nursing role. We started Hera because we felt the system was letting down the generation that raised us. Hera is dedicated care management in the community for older adults, covered by Medicare. Every family gets matched with a dedicated Hero, you, who actually has the tools, time, and autonomy to help. It’s not a 15 min monthly check-in call. It’s real, ongoing support where you see things through and families call you their Hero. The Role: - 1099, 30–40 hrs/week in business hours EST (you set your schedule). - $30–60 / hour: factors include experience, geography, and client load. - Serve older adults and their families in aging at home safely. Qualifications - Hold an LMSW, LCSW, or RN (not required but a plus). - Have experience in geriatric care management, hospice, palliative care, discharge planning, rehab, or hospital social work. - Love fostering connection with older adults and their families. - Are a creative problem solver who wants a job that looks different every day. - Are tech-savvy and excited by quickly evolving environments and leveraging AI to supercharge your work. - Remote - must be located in the United States. A doctor refers a family to Hera. They're cautiously optimistic but mostly overwhelmed - they've been bounced between systems, handed pamphlets, told to "call this number." They don't know what geriatric care management is. They definitely don't know it's covered by Medicare. They're one confusing phone call away from saying "never mind, we'll figure it out ourselves." You're the reason they don't. As an Onboarding Guide, you're the first voice a family hears from Hera. You help them understand how Hera competently cares for their parents, walk them through every step of getting started, and hand them off to their Hero feeling confident, heard, and ready. You're part closer, part concierge, part care coordinator. You just make the path so clear and the experience so easy that saying yes feels obvious. This role sits at the intersection of relationship-building and operations. You're selling, but what you're selling is something families genuinely need, and your job is to remove every obstacle between a referral and a first appointment. Missing paperwork, skeptical adult children, a patient who's never used a computer - you will get through whatever it takes so that the family can meet their Hero. Your mission as the Onboarding Guide - You're the reason families actually get help instead of falling through the cracks between referral and care - You aren’t just going through a motion. You're building trust with real families at a vulnerable moment - You own the full onboarding experience, no handing things off and hoping for the best - You leverage technology to make onboarding seamless, not a stack of faxes and voicemails - Elevate your ceiling. This is not a typical physician role. If you've felt that traditional healthcare pressures clinicians to churn volume and don't actually shape patient outcomes, Hera is for you. An older adult gets discharged from the hospital—you already know what happens. The family scrambles. Nobody talks to each other. Medications get confused. A fall happens. Another admission. The system wasn't designed for this moment. We started Hera because we felt it wasn't enough. Hera is dedicated care management in the community for older adults, covered by Medicare. Every family gets matched with a dedicated Hero (care manager) who has the tools, time, and autonomy to deliver ongoing support. You're the clinical entry point—the comprehensive assessment that sets the clinical foundation for real, ongoing intervention. The Role We're building the clinical infrastructure that makes great geriatric care exceptional. As a Clinical MD, you'll conduct telehealth assessments that serve as the clinical gateway for families newly enrolled with Hera. Each 45-60 minute visit establishes the clinical foundation: confirming Medicare eligibility, synthesizing complex medical and social history, identifying barriers to treatment, and flagging clinical priorities for our care managers (Heroes) to address over months of ongoing coordination. This assessment work directly shapes the care strategy that families receive. Key Responsibilities • Conduct comprehensive telehealth assessments for newly enrolled Medicare clients • Synthesize complex medical histories and identify clinical priorities for ongoing care coordination • Document clinical findings that inform care manager (Hero) intervention strategies • Provide clinical oversight and collaboration as needed with care management team • Establish clinical credibility and trust during initial patient contact Why clinicians tell us Hera feels different • Clinical autonomy. It’s real, ongoing support where you see things through. The Role We're building the clinical infrastructure that makes great geriatric care exceptional. You'd be one of the founding members of our clinical team in new states, shaping how we assess, support, and coordinate care as we launch and grow. As a clinical NP/PA/MD, you'll conduct telehealth assessments that set the foundation for months of coordinated geriatric care. Each 45–60 minute visit is a clinical gateway: confirming Medicare eligibility, synthesizing complex medical and social history, identifying the barriers to treatment, and establishing the trust that families need to engage fully. This is clinical work that matters. Every assessment informs the entire care strategy. Requirements - Licensed NP, PA, or MD (must be able to practice in one of the following states: NJ, MD, PA, MA, CA, FL) - Substantial experience in geriatric primary care, transitions of care, or chronic disease management - Confident in telehealth assessment and skilled at building trust and clinical rapport across a screen - Strong clinical judgment, excellent communication, and comfort with complexity and ambiguity - Aligned with our mission: you see older adults as deserving of excellent, attentive care Compensation - $65 - $175 / hr depending on license type, location and years of experience Hours/Schedule - 3–10+ hours per week to support case load of 40+ clients, flexible - Weekday 11am–4pm ET preferred but can self-select working hours
