Triage Coordinator Location: North Carolina, United States Remote Job Description: Overview Expand Access. Coordinate Care. Support Patients. The Triage Coordinator plays a vital role in supporting after-hours patient care by managing calls related to durable medical equipment (DME), escalating visit needs, and scheduling field nurse visits. Role Description The Care Transitions Clinical reports to the VP of Care Coordination and supports the Director of Care Transitions in proactively identifying patients potentially suitable for hospice care. You will utilize system knowledge, experience, and documentation to educate end users to reduce recurrent issues and implement direct system updates to support resolution and large-scale process changes. This work-from-home telephonic role helps patients and families navigate post-acute services, handles non-clinical inbound calls, escalates to nurses when necessary, and supports referral coordination for patients who may need Gentiva services. Reporting to the Contact Center Manager, this role operates under established performance criteria.
