The Dietitian provides nutrition consultation services to Compassus home health, hospice, and palliative care programs when nutritional concerns require intervention(s). Services are delivered remotely through clinical record review and care team-provided assessment information, with recommendations documented and communicated through approved electronic medical record system. Position Specific Responsibilities - Attends and participates in staff meetings, educational programs and community events, as requested. - Communicates identified needs and potential solutions to supervisor. - Upon receiving a request from the home health, hospice, or palliative care interdisciplinary team (IDT), performs a nutrition consultation aligned with the plan of care and patient/family goals using clinical record review and care team-provided assessment information. - Consults with the IDTs on relevant clinical context from the referring clinician/IDT member as needed to complete the consultation and support nutritional recommendations. - Provides diet education materials and nutrition recommendations to the referring clinician to support implementation of the plan of care. When appropriate permissions and workflows are in place, education materials may also be provided to the patient and/or caregiver through approved channels. - Maintains and documents communications, assessment findings, and nutritional recommendations within the electronic medical record in accordance with Compassus standards. - Develops and maintains standardized nutritional education, resources, and related clinical tools as assigned. - Provides nutrition-related consultation support and guidance to Compassus programs, as assigned. - Maintains availability to support timely consult response and related initiatives. - Maintains required credentials, continuing education, and multi-state licensure/credentialing for the states and programs supported. - Performs other duties as assigned. Role Description The Weekend Patient Transition Coordinator supports seamless care transitions by coordinating referrals to hospice, home health, home infusion, and palliative services. This role works closely with Care Transition Coordinators and Home Health Care Transition Coordinators to ensure patients receive timely and appropriate services. Weekend Hospice Admissions Coordinator

Role Description The Weekend Hospice Admissions Coordinator processes patient referrals, coordinates the admission of patients, and enters patient admission data after all requirements of hospice have been met. Weekend schedule: Thursday, Friday, Saturday, Sunday. Thursday and Friday 11am-8pm and Saturday and Sunday 9am-8pm. This is a remote role, but you MUST be local to Pennsylvania. LPN strongly preferred. Hospice admissions and referral experience highly desired. Position Specific Responsibilities - Support designated program areas and/or regions. This role will support multiple regions. - Attends and participates in staff meetings, educational programs and community events, as requested. - Communicates identified needs and potential solutions to supervisor. - Receives and processes referrals from physicians, patients, families, friends, and community. - Contacts physicians to secure an order for admission. - Contacts insurance companies for pre-certification authorization. - Schedules appointments with patients and families for informed consents and the RN assessment visit. - Explains hospice services and benefits to patients, family members, and significant others, if needed. - Informs appropriate team members of time of appointments. - Completes admission process according to agency/corporate policy. - Ensures proper notification of admission to assigned interdisciplinary team (IDT) and ancillary providers. - Enters admission data into a third-party system. - Ensures patient chart, ensuring all documentation is signed and dated and turns chart over to Team Coordinator. - Markets referral sources. - Contacts long term care staff, hospital staff, and physicians to promote hospice services. - Provides back-up support for Hospice Care Consultant. - Documents marketing contacts, as directed. - Performs other duties as assigned. Revenue Cycle Specialist I

Role Description The Revenue Cycle Specialist I is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Revenue Cycle Specialist I is responsible for generating accounts receivable revenue, processing claims, following up on claims, maintaining collection notes, posting deposits, submitting provider notifications, and balancing all aspects of accounts receivable. S/he also provides backup for accounts payable when accounts receivable revenue is involved, which includes working with hospitals and facilities to ensure accurate patient information. S/he is responsible for knowing and applying the rules and regulations of state and federal regulatory agencies and other certified agencies. Qualifications - High school diploma or GED required. - Experience in medical health care collections required, home health, hospice, palliative, or infusion collections preferred. - Knowledge of Commercial, Third-Party Insurance Accounts, including but not limited to Medicaid, Managed Care, HMO, PPO, Auto and Work Comp rules and guidelines governing collection activities highly preferred. Requirements - Collects outstanding claims. - Provides support and consultation to both corporate and branch personnel. - Attends and participates in staff meetings. - Communicates identified needs and potential solutions to Senior Specialist/Supervisor. - Reviews and submits Notice of Elections and Notice of Revocation/Terminations as required for payors. - When applicable, reviews new admissions to verify payors and benefit periods. - Runs pre-billing reports on assigned branches and pre-billing claims for review. - Monitors and assists program branches as needed with financial classes and recertification dates. - Generates Patient Revenue. - Reviews and transmits/mails claims. - Maintains knowledge of appropriate data required on claims. - Assures filing of accurate claims according to payer specifications. - Submits appropriate claims corrections or adjustments as needed. - Documents required back up/approvals as required on revenue adjustments/changes and submits it to their Supervisor/Manager. - Assembles required back up/approvals as required on revenue that will result in bad debt and submits it to their Supervisor/Manager. - Works with branches, hospitals, and long-term care facilities as needed to ensure accurate billing and accounts payable information. - Able to self-organize work assignments to assure efficiency and effectiveness. - Generates unbilled revenue reports at end of month and confirms all patients that were not generated are accounted for. - Works the unbilled revenue report weekly by communicating missing information to the branches to ensure generation of all revenue. - Maintains up-to-date knowledge of Insurance Contracts and notifies Senior Specialist/Supervisor/Manager in writing of any problems or deviation from contract. - Maintains up-to-date knowledge of Medicare, Medicaid, and Private Insurance rules/procedures/and rates for billing. - Monitors every outstanding claim on the aging report at least once every two weeks. - Ensures that any account over 60 days old is being investigated on a consistent basis and documents every follow up attempt. - Monitors for ADR’s, or other payor request, needed to complete billing/collection process, on a weekly basis and notifies appropriate personnel. - Performs quarterly status checks on all current ADR patients. - Works Returned to Provider (RTP) claims in Medicare. - Monitors Medicaid and Private Insurances websites, clearinghouses, and RA/EOB’s on a daily basis to ensure that all claims are processing for payment. - Performs monthly AR reviews and works with Senior Specialist/Supervisor/Manager to resolve problem accounts. - Posts Deposits daily or weekly as needed. - Balances EOB/RA’s to deposit entry and bank printouts. - Assists with preparation and delivery of items required for branch or yearly audits as needed. - Follows timekeeping and attendance policy daily. - Performs other duties as assigned.
