• The Membership Associate processes and maintains documentation related to PO participation and health plan enrollment. • Inputs and updates client data in the customer relationship management software. • Maintains accuracy of membership roster for select physician organizations. • Independently administers several assigned responsibilities, ensuring excellent customer service for both internal and external clients. • Submit and monitor practice and physician applications and terminations; follow up as needed. • Collaborate with practice coaches to obtain accurate data for enrollment. • Compile and maintain current and accurate data for all providers in databases and online systems. • Maintain strict confidentiality of sensitive physician information and membership documentation. • Communicate with practices on a regular basis for physician updates, information requests, and general upkeep. • Communicate with health plan representatives on outstanding issues. • Collaborate with Market Operations Managers and Operations Consultants to reconcile physician lists and administrative payments. • Work alongside the membership team and Operations Manager to streamline and optimize current processes. • Attend mandatory and routine meetings as required. • Perform other duties as assigned. Maintain flexibility to pivot and meet shifting or dynamic business needs.
• Optimize patient outcomes through telephonic interventions • Focus on the tactical execution of medication management services, including polypharmacy, adherence, and pharmacotherapy optimization • Counsel patients via telephonic outreach on medication adherence, address any medication related problems such as adherence barriers, polypharmacy, or high risk medication use, and make appropriate evidence-based recommendations to patients and Practice Care Teams • Perform focused clinical chart reviews as needed to inform patient interventions • Obtain and document patient commitment to medication adherence and address barriers in real time • Escalate complex clinical cases to prescribers or interdisciplinary teams as appropriate • Meet and sustain performance goals (e.g., STARs, HEDIS) • Collaborate with Physicians, clinical and administrative staff to optimize the Medication Management workflow and patient experience • Proactively identify gaps in medication management through patient interactions and translate patient insights into targeted recommendations that drive improvements in practice operations and clinical workflows • Participate in the design, implementation, and evaluation of pilot programs or process changes (“test and learn”) within the outreach model • Serve as a clinical subject matter expert, leveraging clinical pharmacy expertise to support organizational objectives and adapt to evolving healthcare needs • Develop and maintain standardized resources, educational tools, and clinical protocols to support improved medication outcomes and performance on quality measures (e.g., STARS, HEDIS) for practices, clinicians, and patients
• Conduct outbound prospecting activities, including cold calling, emailing, and social selling, to generate leads within the primary care sector • Qualify leads by understanding practice needs and assessing their fit for Aledade's value-based care model • Collaborate with the sales team to develop targeted outreach strategies and campaigns, while maintaining accurate records of all prospecting activities in our CRM system and activity system outreach.io • Meet and exceed monthly and quarterly quotas for qualified lead generation • Stay updated on industry trends and Aledade's solutions to effectively communicate value propositions to prospects
• Develop and execute the comprehensive GTM launch blueprint for all new and renewing value-based care contracts • Lead and build a high-performing team. Set priorities, drive accountability, and foster a culture of cross-team collaboration and continuous improvement. • Construct explicit implementation timelines, gating milestones, and operational workflows mapping out the journey from a signed payer contract to "active" status in the field. • Build, update, and manage cross-functional tracking dashboards using project management platforms (e.g., Monday.com, SalesForce) • Author market-specific "Payer Snapshots” that local field or sales teams can use to pitch independent primary care providers (PCPs). • Deconstruct complex payer contracts into clear, simplified sales enablement collateral. • Partner with Analytics, Outreach (practice growth), Provider Networks, and Strategic Payer Partnerships to ensure alignment of priorities and execution across multiple workstreams and timelines. • Create Training Program for field teams to ensure fluency on new payer products • Establish a market feedback mechanism around physician friction points and identify solutions. • Establish KPIs and reporting mechanisms to track enrollment growth. • Continuously analyze the sales pipeline to identify opportunities and adjust launch strategies. • Other duties as assigned
• Place outbound calls using the Five9 dialer to schedule patient appointments for wellness visits, follow-ups, and other care needs. • Follow standardized scripts and workflows to guide patient conversations and ensure consistency in messaging. • Schedule appointments directly in the appropriate system or coordinate with the practice to complete bookings. • Handle inbound return calls and voicemails, assisting patients with scheduling or routing them to appropriate teams. • Document all call activity and outcomes in real time using Five9 and internal tracking tools. • Verify and update patient contact information during calls to maintain data accuracy. • Transfer or escalate calls requiring clinical input or coordination beyond the scope of the role. • Educate patients on available services and appointment types, answering basic questions to support engagement • Other duties as assigned
• As an Analytics Technical Product Manager you will be a key member of the Analytics organization, the team that delivers insights and decision making tools to drive Aledade strategy and support our various lines of business. • In this role, you will report to an Analytics leader and serve as a force multiplier for our teams. • You will work alongside business owners, product team members, data analysts, designers and engineers to define project goals, solution scope, implementation approach, and rollout plans. • You will play a significant part in scoping analytics work, managing stakeholders, and delivering high-quality, high-impact analytics products with the ability to scale. • You will support teams of Analytics professionals to help us deliver new analysis and complex data products that power intelligence across our suite of tools.
• Identify, analyze, and resolve clinical data quality issues across Aledade's multi-vendor EHR ecosystem. Investigate root causes of data discrepancies at the interface, configuration, and source system level. Produce structured findings with actionable recommendations for engineering and integration teams. • Map clinical data elements to standardized code systems and validate mapping accuracy across clinical domains. Support the development and maintenance of Aledade's terminology mapping capabilities, including rule-based and AI-assisted approaches. • Support quality measure programs by ensuring clinical data completeness and accuracy throughout the reporting pipeline. Validate measure logic and code sets against published specifications. Identify data gaps impacting measure performance and collaborate cross-functionally to resolution. • Serve as the clinical subject matter expert for new data sources, AI-powered extraction, and interoperability initiatives. Provide clinical context to engineering, product, and operations teams for data governance, vendor engagement, and process improvement efforts.
• Working cross functionally to design, build, and operate solutions that continuously improve and automate our security capabilities • Leveraging data to understand trends, metrics, and opportunities to improve our security posture and then helping execute on those opportunities with stakeholders • Leading and enhancing incident response efforts, spearheading analysis, containment, and mitigation strategies in a cross-functional environment to ensure effective resolution and remediation of security incidents • Helping craft and refine security documentation pertinent to our Security Program, such as policies, standards, baselines, and standard operating procedures • Mentoring and coaching more junior engineers or analysts
