GUIDE Program Coordinator

• Guide patient and their caregivers through the onboarding process after enrollment • Monitor completion of dementia screening and health assessments • Coordinate tablet shipment and provide orientation to the GUIDE program • Schedule and track formal neurology evaluations with our clinical partners • Review documentation and prepare applications for CMS submission • Track progress and report status updates to internal teams • Ensure a warm handoff to Engagement Specialists and Coaches • Collaborate on software workflows • Adapt to evolving processes and contribute to continuous improvement

Job Requirements

  • Bachelor's degree in healthcare administration, social work, public health, or related field preferred
  • Experience in patient navigation, care coordination, or healthcare customer service
  • Strong interpersonal and communication skills
  • Detail-oriented with excellent organizational and time management abilities
  • Comfortable working in a fast-paced, tech-enabled environment
  • Demonstrates flexibility in response to changes in workflows, tools, and program requirements.
  • Familiarity with CMS programs or dementia care is a plus
