Role Description The Cardiac Rehab Onboarding EP is a patient-facing role responsible for onboarding, education, and early engagement of patients enrolled in the Intensive Cardiac Rehabilitation program after the initial session. This role supports patients during the first sessions by providing goal setting, program education, progress monitoring, and technical assistance to ensure patients can successfully participate in both individual and group sessions. The Onboarding EP works collaboratively with a multidisciplinary care team, including nurses, exercise physiologists, and patient experience specialists to support patients across various stages of recovery following a cardiac event.
