Role Description The Cardiac Rehab Onboarding EP is a patient-facing role responsible for onboarding, education, and early engagement of patients enrolled in the Intensive Cardiac Rehabilitation program after the initial session. This role supports patients during the first sessions by providing goal setting, program education, progress monitoring, and technical assistance to ensure patients can successfully participate in both individual and group sessions. The Onboarding EP works collaboratively with a multidisciplinary care team, including nurses, exercise physiologists, and patient experience specialists to support patients across various stages of recovery following a cardiac event. Key Responsibilities - Patient Onboarding & Education - Conduct first session and early follow-up sessions to orient patients to the cardiac rehabilitation program including completing fitness testing - Educate patients on program structure, expectations, and participation requirements, including group and individualized sessions - Reinforce the importance of regular attendance, engagement, and completion of the program - Support patients in understanding next steps in their care journey - Technical Support & Session Readiness - Assist patients with basic device and platform troubleshooting - Guide patients step-by-step on how to join virtual sessions (audio, video, platform navigation) - Ensure patients are technically prepared to independently access future sessions - Identify and escalate unresolved technical issues to the appropriate team - Goal Setting & Behavioral Coaching - Support patients in establishing realistic, personalized health and recovery goals - Provide motivation and emotional support, addressing common barriers such as anxiety, low confidence, or disengagement - Utilize basic behavioral and coaching techniques to promote adherence and sustained participation - Group Engagement Support - Enroll patients into appropriate group education sessions - Educate patients on the value and expectations of group participation - Identify and address barriers to group session attendance (technical, scheduling, understanding) - Collaboration & Documentation - Communicate patient progress, challenges, and concerns to the care team as needed - Maintain accurate, timely documentation of sessions, goals, and patient interactions - Record relevant data for program tracking and quality improvement efforts Qualifications - Bachelor’s or Master’s degree in Health, Exercise Science, Kinesiology, or a related field - CPR and First Aid certification (required) - ACSM Certification- Clinical Exercise Physiologist/ Exercise Physiologist or AACVPR-CCRP (required) Requirements - Experience working in a clinical, rehabilitation, or patient support setting preferred - Background in health coaching, fitness, or exercise physiology strongly preferred - Experience supporting cardiac or chronic care populations is a plus - Experience with motivational interviewing, patient engagement and customer service. Core Skills & Attributes - Strong communication and patient education skills with an ability to build rapport with patients in a short time - Comfort providing basic technical support in a virtual care environment - Knowledge of cardiovascular health, behavior change principles, and lifestyle modification - Empathy, patience, and ability to support patients with varying physical and emotional needs - Strong organizational skills and ability to work independently in a remote setting Role Summary This role is designed to reduce friction at program entry, improve early patient engagement, and support smooth transitions into ongoing cardiac rehabilitation care by combining patient education, coaching, and technical readiness. Role Description We’re seeking a Patient Growth Specialist to support the expansion of our virtual cardiac rehabilitation program. This role is ideal for candidates with a clinical or exercise science background who are excited to combine patient education with performance-driven outreach. - Engage cardiac patients through high-volume outbound calling. - Educate patients on the benefits of cardiac rehabilitation, address clinical and logistical questions, overcome hesitation, and guide patients through enrollment and initial technical setup. - This is a highly conversion-focused role that blends clinical credibility with strong sales and objection-handling skills. Key Responsibilities - Make high-volume outbound calls to eligible cardiac patients identified through health system partners. - Educate patients on cardiac rehabilitation, program expectations, and clinical benefits in a clear, reassuring way. - Use strong objection-handling and motivational interviewing techniques to drive enrollment and attendance. - Answer basic clinical questions within scope and appropriately escalate complex clinical concerns to the care team. - Assist patients with mobile app downloads, account setup, and basic technical troubleshooting. - Complete appointment reminder and confirmation outreach to reduce no-shows and improve attendance. - Accurately document call outcomes, patient status, and next steps in internal systems. - Meet or exceed daily call volume, enrollment, and attendance targets. - Partner closely with Enrollment Operations and Clinical teams to ensure a high-quality, compliant patient experience. Qualifications - Bachelor’s degree in Exercise Science, Kinesiology, Health Sciences, Nursing, or a related clinical field or equivalent clinical experience in cardiac rehab, physical therapy, or patient care. - Background in one or more of the following: - Exercise physiology - Cardiac rehabilitation - Clinical care coordination - Patient education or health coaching - Strong verbal communication skills with the ability to build trust quickly with cardiac patients. - Comfort discussing cardiac conditions, procedures, and recovery in patient-friendly language. - Proven ability to perform in a metrics-driven, high-volume calling environment. - Strong objection-handling skills and ability to motivate behavior change. - Technical comfort helping patients navigate mobile apps and resolve basic tech issues. - Reliable internet connection and a quiet, professional home workspace. Highly Preferred Experience - Certified Exercise Physiologist (ACSM-CEP), Clinical Exercise Physiologist, Cardiac Rehab Specialist, RN, or similar clinical credentials. - Prior experience in: - Cardiac rehab programs - Hospital discharge or referral coordination - Patient navigation or care management - Experience enrolling patients into clinical or digital health programs - Familiarity with EMRs, CRMs, power dialers, or patient engagement platforms - Sales, outreach, or growth experience in healthcare or digital health environments Compensation $30/hour Work Structure Fully Remote (United States) Hours/Schedule Monday–Friday, 10:00 AM – 6:00 PM ET Reporting to Senior Enrollment Operations Manager Location United States Note This is a 1099 contractor position.
