• The Case Management Coordinator utilizes critical thinking and judgment to collaborate and inform the case management process • The Case Management Coordinator facilitates appropriate healthcare outcomes for members by aiding with appointment scheduling, identifying and assisting with accessing benefits and education for members through the use of care management tools and resources • Evaluation of Members: Conducts comprehensive evaluation of referred member’s needs/eligibility and recommends an approach to case resolution • Identifies high risk factors and service needs that may impact member outcomes and care planning components • Coordinates and implements assigned care plan activities and monitors care plan progress • Enhancement of Medical Appropriateness and Quality of Care: Consults with case managers, supervisors, Medical Directors and/or other health programs to overcome barriers to meeting goals and objectives • T Provides coaching, information and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices

• Monitor public social platform discussions related to WES services • Identify high visibility legacy threads and emerging discussions • Track recurring questions or misunderstandings in community conversations • Provide clear, factual responses to general questions about WES processes • Correct misinformation or outdated narratives about WES • Assess sensitivity and reputational risk before engaging in discussions • Guide users toward official WES documentation and support channels • Collaborate closely with Customer Service leadership to ensure alignment • Maintain a credible, respectful, and professional presence representing WES

• Build and maintain relationships with local schools, PTAs, sports teams, and community organizations • Identify and pitch fundraising opportunities to local businesses and organizations • Collaborate with store managers to ensure events are properly scheduled and resourced • Reach out to potential partners via direct messages on social media platforms (Facebook, Instagram, etc.) • Make outbound calls to schools and businesses to introduce partnership opportunities • Send professional, well-crafted messages via email and social channels • Coordinate School Pizza Nights and other fundraising events • Organize and market community events, including table tents, spirit nights, and donation requests • Track outreach efforts, partnership status, and event results in a simple CRM or spreadsheet • Report weekly on outreach activity and partnership pipeline
