• Managing day-to-day operations for in-home service delivery • Overseeing client and staff scheduling to ensure consistent, high-quality care • Supporting and onboarding Behavior Technicians working in the field • Partnering with Clinical Leadership to maintain smooth service coordination • Assisting with community outreach and local relationship-building • Serving as a point of contact for families to ensure a positive service experience

• Assesses, plans and implements care strategies that are individualized by member and directed toward the most appropriate and least restrictive level of care. • Collaborates with member, family and healthcare providers to develop an individualized plan of care. • Conducts individual counseling and group therapy with adolescents and adults. • Identifies and initiates referrals for social service programs – including financial, psycho-social, community and state supportive services. • Advocates for members and families as needed to ensure the patient’s needs and choices are fully represented and supported by the healthcare team. • Utilizes approved clinical criteria to assess and determine appropriate level of care for members. • Documents all member assessments, care plan and referrals provided. • Responsible for achieving set goals; Key Performance Indicators (KPIs). • Learning the StrongWell model and taking responsibility and ownership for outcome based care. • Participates in interdisciplinary team meetings and utilization management rounds and provides information to assist with safe transitions of care. • Promotes responsible and ethical stewardship of company resources. • Maintains excellent punctuality and attendance during work hours.

What You’ll Do - This is a non-clinical, operational role that partners closely with Sleep Technologists during the transition from treatment setup to long-term resupply - Coordinate follow-up, execution, and documentation to ensure a smooth and reliable experience once patients enter the resupply phase - Work within a tech-enabled, AI-supported care environment - Collaborate with clinical, billing, and operations teams - Help track engagement and reorder metrics while contributing to a scalable operational foundation inside a fast-growing digital sleep clinic - Execute timely and proactive resupply outreach - Follow structured retention workflows - Maintain accurate reorder tracking - Communicate clearly and compassionately with patients - Escalate clinical or billing issues appropriately - Support adherence monitoring efforts Qualifications - 2+ years in healthcare coordination, DME, or care navigation - Experience with PAP/CPAP resupply (strong plus) - Strong patient communication skills with a high level of empathy and professionalism - Ability to work autonomously in a fast-paced, remote environment while collaborating effectively with cross-functional teams - Confidence navigating digital tools and care platforms - Data driven mindset with the ability to track tasks, maintain accurate records, and follow structured workflows - Comfort managing proactive outreach and patient follow-up while maintaining strong attention to detail - Genuine commitment to improving patient experience, therapy adherence, and continuity of care - Operational and automation mindset - Availability to work Central Time (CST) business hours (preferred) Benefits - Meaningful work that directly improves peoples’ lives - Be part of an international team across the US, France, Belgium - Annual team offsite - Comprehensive health benefits (medical, dental, vision) - FREE One Medical membership - 401(k) with company match - 20 days PTO + 10 paid holidays + 80 hours paid sick leave - Internet stipend Compensation $25 - $30 per hour depending on experience.
