Nurse Case Manager

Role Description L’infirmier(ère) chargé(e) de la gestion des cas (ICGC) jouera un rôle essentiel dans le soutien aux patient(e)s, en fournissant un soutien au remboursement aux médecins, ainsi que des services de gestion des cas médicaux aux patient(e)s. Aide le (la) médecin à remplir tous les documents spécifiques au programme et assure le suivi des demandes relatives à l’obtention d’un remboursement ou d’une couverture du médicament par des formulaires de couverture privée et provinciale, afin de s’assurer que les patient(e)s aient accès en temps opportun aux traitements qui leur ont été prescrits. Sert de ressource aux médecins qui prescrivent ce médicament et offre un soutien continu aux patient(e)s pour le suivi, les renouvellements et les changements de couverture. Duties and Responsibilities - Aider à l’inscription des patient(e)s, notamment en recueillant tous les renseignements pertinents et appropriés pour remplir les formulaires nécessaires (assurance privée et/ou provinciale) de façon concise, organisée et professionnelle. - Assurer la liaison et plaider auprès des fournisseurs tiers et des gouvernements provinciaux, au besoin, pour déterminer les options de couverture. - S’assurer que les patient(e)s ont rendez-vous pour les examens diagnostiques nécessaires et informer les médecins de tout résultat d’examen périmé nécessitant un suivi. - Accomplir les activités d’appel de bienvenue avec le (la) patient(e) selon des scénarios définis. - Fournir aux patient(e)s et aux médecins un numéro sans frais direct. - Examiner les rapports après la visite pour confirmer que le prochain rendez-vous est programmé, et vérifier l’intégrité des données du rapport par rapport à l’ordonnance du (de la) médecin, le cas échéant. - Trouver les rapports après la visite manquants, le cas échéant. - Faciliter toute communication requise avec les médecins, les programmes de soutien aux patient(e)s ou les autres professionnel(le)s de la santé impliqué(e)s dans le cercle de soins du (de la) patient(e). - Assurer le suivi et la défense des intérêts jusqu’à ce qu’une décision soit prise, idéalement en obtenant une couverture pour le (la) patient(e), le cas échéant. - Travailler en étroite collaboration avec les représentants commerciaux locaux du fabricant pour répondre aux besoins spécifiques des médecins et du personnel, le cas échéant. - Proposer et/ou dispenser une formation aux patient(e)s et aux médecins sur le(s) produit(s). - Agir à titre de personne-ressource centrale pour la liaison avec les parties prenantes du programme, notamment les médecins, les infirmier(ère)s, les patient(e)s et les cliniques. - Coordonner et planifier les visites des patient(e)s dans les lieux appropriés (domicile, clinique, etc.), le cas échéant. - Fournir des mises à jour aux parties prenantes internes et externes sur la base des observations. - Veiller à ce que les dossiers des patient(e)s soient tenus à jour et que les données soient exactes afin de répondre aux exigences du programme en matière de rapports. - Assurer une déclaration rapide et conforme des événements indésirables (EI) et des réclamations qualité produit (PQC) conformément aux exigences et directives de pharmacovigilance de Santé Canada, incluant une documentation précise, une escalade appropriée et une collaboration efficace avec les parties prenantes internes et les équipes réglementaires. - Participer aux activités de qualité et aux initiatives d’amélioration continue conformément au système de gestion de la qualité de l’entreprise ainsi qu’aux indicateurs de performance spécifiques au projet. - Préserver la confidentialité des renseignements relatifs aux partenaires pharmaceutiques et à l’entreprise, et n’en discuter qu’avec le personnel approprié de Bayshore. - Remplir tous les rapports pertinents (feuilles de temps, dépenses, kilométrage, rapports, etc.) dans les délais impartis et conformément à la politique de Bayshore, le cas échéant. - Remplir les rapports sur les plaintes relatives aux produits. - Adhérer aux politiques et aux procédures de Bayshore et du fabricant. - Effectuer d’autres tâches, selon les besoins. Qualifications - Diplômé(e) d’un Programme d’infirmier(ère) autorisé(e) ou d’un Programme d’infirmier(ère) auxiliaire autorisé(e)/immatriculé(e), titulaire d’une autorisation d’exercer en vigueur délivrée par un organisme provincial de réglementation au Canada. - Inscrit(e) et en règle auprès de l’organisme provincial de réglementation. - Au moins deux ans d’expérience dans des Programmes de soutien aux patient(e)s ou l’équivalent. - Expertise démontrée dans la prestation d’un service à la clientèle exceptionnel et dans l’établissement de relations positives avec les parties prenantes internes et externes. - Maîtrise de la communication verbale et écrite et des compétences interpersonnelles en anglais et en français (pour les provinces/régions ayant des exigences de service en français). - La compréhension des critères de couverture provinciaux et privés, des processus d’autorisation spéciale et de la façon de s’orienter dans le paysage de la couverture des médicaments au Canada est un atout. - Compétences informatiques et techniques solides : Excel, Word, Outlook, tablettes électroniques, portails Web/utilisation d’Internet. - Excellentes compétences en matière de réflexion critique et de résolution de problèmes. - Capacité à prendre des décisions en faisant preuve de discernement. - Aptitude à collecter des données de manière précise et à assurer leur intégrité. - Facilité à travailler dans un environnement sans papier. - Capacité à se dépasser pour obtenir des résultats. - Capacité avérée à travailler en équipe. - Capacité à résoudre les conflits de manière professionnelle. - Sens aigu de l’organisation et souci du détail. - Gestion autonome du temps et capacité à effectuer plusieurs tâches simultanément tout en respectant les délais. - Connaissance de la LPRPDE et de son application dans un environnement confidentiel lié aux patient(e)s. - Aptitude à travailler dans un environnement de travail calme avec une capacité avérée à travailler de manière autonome au sein d’une équipe dans un environnement de télétravail/virtuel. - Accès Internet haute vitesse établi au bureau à domicile.
