Role Description Responsible for nutritional care of patients. Plans, assesses, implements, and evaluates patient nutritional care plan. Assists in menu modifications and other nutritional related functions. - May mentor and train team members. - Responsible for accurately and responsively performing varied clinical tasks and problems of minimal complexity. - Interviews patient and/or family for nutritional history, obtains patient food preferences, assesses patients, and documents in patients’ medical record. - Conducts nutritional assessment, develops and implements patient specific care plans, and assists in recommending nutrition care plans to medical staff for review and consideration. - Recommends therapeutic diet modifications and/or supplemental feedings to physicians for patients, and ensures accuracy of diet order received by checking against physician’s order. - Provides nutritional guidance, education and counseling to patients and their families in accordance with the prescribed diet. Calculates nutrient content of tube feeding and dietary intakes as ordered by physicians. - Contributes to changes to patient’s menu, including patient food preferences, and the development of standardized recipes, as well as assists in overseeing food service workers in proper food handling and portion control. - Supports a clinical function to assist clinical team members and department or unit management with carrying out daily clinical operations. - Prepares, presents, and conducts individual and group education classes, and refers clients to community services, as needed. Participates in and promotes Northwell wellness initiatives. - Develops and utilizes effective lines of communication regarding nutrition information within the program and community. - Provides in-services to culinary, food service and interdisciplinary teams regarding therapeutic diets and related topics. - Participates in Quality Assurance/Performance Improvement related tasks including data collection and executes tasks related to it, as directed. - Solves or escalates operational problems and technical issues to senior team members and management; modifies existing tools or processes. - Performs related duties as required. Qualifications - Master's Degree or equivalent combination of education and related experience, required. - Current Registered Dietitian Certification with the Commissions on Dietetic Registration, required. - Certified Dietitian Nutritionist in New York State (CDN), as needed. - Advanced/specialized certifications, as needed. - 1-3 years of relevant experience, required. Role Description As a Registered Nurse with Best Home Health Providers, you will play a key part in ensuring the accuracy and quality of clinical documentation, helping our patients receive the right care at the right time. You’ll collaborate closely with clinical staff, contribute to quality improvement initiatives, and help uphold our commitment to regulatory compliance. - Review OASIS assessments for completeness, appropriateness, and compliance with federal and state regulations. - Ensure accurate ICD-10 coding and sequencing based on each patient’s medical condition and co-morbidities. - Partner with clinical staff to resolve documentation and data integrity issues. - Monitor visit utilization and flag potential financial risks or underutilization to clinical management. - Identify and communicate trends that impact agency outcomes and process measures. - Conduct OASIS inter-rater reliability visits with clinical team members. - Participate in Quality Improvement and Corporate Compliance activities. - Stay current with best practices through ongoing education and professional development. - Perform other duties as required for patient services. Qualifications - Active Californian RN license in good standing. - 1–2 years of clinical home health experience. - Working knowledge of OASIS assessments and ICD-10 coding. - Knowledge of federal regulations and state licensure requirements. - Strong communication skills and a keen eye for detail. - Ability to work independently and manage priorities with minimal supervision. - Proficiency with computer systems and electronic health records. - Current CPR certification with AHA. - Valid driver’s license, reliable insured vehicle.
