• Following departmental policies to collect data for the verification and evaluation of medical staff credentials. • Obtain primary source verification of professional credentials for applicants to the medical staff and practices in accordance with departmental policy. • Collect data and prepare medical staff reappointment files for presentations to various committees and physicians. • Maintain the confidentiality of highly sensitive practitioner-specific information. • Maintain accurate, complete and timely information in the credentials software system. • Assist in the maintenance of systems to track expiring documents. • Assist in the preparation and updating of all required documents. • Use discretion in preparing reports and releasing such reports containing highly sensitive practitioner-specific information. • Respond to queries regarding status of pending applications. • Other duties as assigned.

• Provide psychiatric evaluations and ongoing medication management for patients across the lifespan (ages 5+). • Develop and implement evidence-based treatment plans that align with patient goals. • Prescribe and manage psychotropic medications, monitoring for efficacy and side effects. • Collaborate with primary care providers and multidisciplinary teams to support whole-person care. • Address co-occurring disorders, including substance use, with a comprehensive, evidence-driven approach. • Maintain timely, accurate documentation in partner-site EHR systems to support care continuity. • Engage patients through telehealth platforms to ensure accessibility and equity in care delivery.

• Provide in-person and telehealth psychiatric care to adolescents (11+) and adults • Conduct comprehensive psychiatric evaluations, develop and implement individualized treatment plans • Prescribe and manage medications, with careful attention to safety, efficacy, and patient education • Maintain timely and complete clinical documentation in accordance with state and practice standards • Participate in case consultations and peer collaboration • Build strong therapeutic rapport with patients from diverse backgrounds
