Role Description Join DaVita Integrated Kidney Care (IKC) as a Care Coordinator. It takes a village to care for our patients, and as a Care Coordinator (CC), you’ll be at the heart of that Village. You’ll be a key player on our care team, helping some of our most complex patients navigate their chronic conditions while ensuring they receive the highest quality care. You’ll work closely with Nurse Practitioners and RN Case Managers to coordinate care, facilitate resources, and keep the patient journey moving smoothly.
