Responsibilities - Effectuer des consultations virtuelles expertes pour les évaluations de la ménopause. - Assurer des suivis complets pour les préoccupations liées à la ménopause. - Élaborer des plans de traitement personnalisés adaptés aux besoins uniques de chaque patiente. - Prescrire et gérer l'hormonothérapie substitutive et autres traitements. - Responsabiliser les patientes grâce à l'éducation sur les modifications du mode de vie et les stratégies d'autosoins. - Collaborer harmonieusement avec notre équipe de médecins dévoués. - Profiter des avantages de notre processus d'admission de pointe. - Maintenir une documentation précise, complète et en temps opportun dans le dossier médical électronique. - Traiter et gérer la documentation entrante des laboratoires et des références. - Répondre aux communications médicales urgentes et transmettre les résultats critiques. - Coordonner avec les laboratoires, les cliniques, les patientes et les équipes internes. - Contribuer à une expérience de soins fiable et de haute qualité. Qualifications - Infirmière autorisée avec un permis actif au Québec requis. - Si non titulaire d'un permis en Ontario, la volonté de faire une demande est requise. - Bilinguisme en français et en anglais requis. - Expérience spécialisée en santé des femmes, en hormonothérapie et en soins holistiques. - Connaissances de base en nutrition, exercice et psychologie. - Familiarité avec les techniques d'entretien motivationnel. - Certification ou formation supplémentaire liée à la ménopause préférée. Role Description The incumbent serves as the Administrative Officer (AO) for the VISN 4 Nurse Clinical Triage Contact Center (RN CCC) and Pharmacy Clinical Contact Center (CCC), supporting virtual care services delivered by triage nurses, pharmacy technicians, and pharmacists. The role provides administrative oversight, coordination, and integration of services within the Clinical Contact Center Modernization initiative and the VISN 4 Clinical Resource Hub. Responsibilities - Plays a critical role in meeting or exceeding performance expectations related to access, patient flow, and timely, efficient, and cost-effective processes throughout the VISN 4 CCC's and local medical centers.
