Role Description The MH FIRST Remote RN - Float is pivotal in providing critical support to field teams facing urgent and complex patient concerns. The Registered Nurse has expertise that will ensure patients receive the care, resources, and support they need, especially during high-stress situations and environmental crises. - Utilize licensing and crisis management skills to assess and address urgent needs. - Assess care plan requirements and assist in their implementation. - Utilize proficiency in documentation and technology to streamline member care. - Offer valuable support and guidance to patients and their families in critical situations. - Primary point of contact for patients, Monogram field staff, and care center personnel needing immediate support. - Provide seamless transitions by providing comprehensive handoffs to incoming staff. - Conduct chart auditing during non-peak call volumes. - Emergency/disaster outreach as events occur. - Support service level agreement (SLA) compliance for annual assessments and care plans. - Assist in post-hospital outreach to ensure patient needs are met, with no outstanding needs that could result in readmissions. • Act as an extension of CDI Division Management by serving as a liaison between CDI, Coding, Quality, HIM and Physician Advisors. • Serve as the initial go-to person and ensures follow-up is completed. • Triage, research, prioritize, and follow up on incoming inquiries. • Work with others as needed to resolve issues and implement solutions. • Promote and maintain standardization of the EHR and CDI workflows across the corporation. • Prioritize and follow up on division initiatives and tracking to help ensure efficiency.

• Provides clinical based review and analysis of the documentation in the patient’s medical record • Supports accurate capture of patient’s complexity and severity of illness • Utilizes queries of medical staff and other caregivers to clarify and support completeness of documentation • Collaborates with physician, mid-level providers, and medical records coders to identify principal and secondary diagnoses • Conducts initial and ongoing concurrent and some retrospective reviews for system wide admissions • Initiates tracking process and documents findings on EHR tracking system • Communicates with providers to capture accurate documentation of patient’s severity of illness

• Demonstrate a strategic and scientific command of assigned therapeutic areas and products • Write original content for medical and promotional communications tactics and activities, including content for disease state symposia, advisory boards, speaker training programs, and other multimedia initiatives • Effectively communicate complex medical and scientific concepts in a clear, engaging, audience-appropriate manner
