Job Description

Caregiver Coach

Ceresti Health

• Deliver high-quality, person-centered coaching to family caregivers of individuals living with dementia, tailored to their unique challenges. • Use Ceresti’s proprietary technology platform to manage daily workflow, track caregiver progress, and document caregiver interactions. • Initiate and maintain consistent outbound engagement to establish trust and build strong, long-term relationships with assigned caregivers. • Educate caregivers on the dementia disease process, common behavioral symptoms, safety strategies, and techniques for managing stress and preventing burnout. • Coach caregivers through emotionally complex scenarios, such as end-of-life conversations, transitions to facility care, or family conflict around care decisions. • Guide caregivers in care planning, appointment preparation, accessing community-based resources and care coordination, helping them communicate effectively with healthcare providers and navigate the healthcare system. • Meet or exceed performance metrics related to caregiver engagement, satisfaction, and program outcomes. • Attend and contribute to regular team huddles, trainings, and case reviews, sharing insights and best practices with peers and leadership. • Participate in quality improvement initiatives, including pilot studies, internal feedback loops, and caregiver experience evaluations. • Other duties as assigned.

Job Requirements

  • 2+ years of experience working professionally with older adults or persons living with dementia.
  • Direct caregiving experience (personal or professional) with persons with cognitive impairment.
  • Fluency in English.
  • Must be eligible to work in the U.S.
  • Ability to work independently in a fully remote setting.
