Specialty Care Navigator, Mood & Anxiety

Spring Health

• Telephonic screening, brief intervention, and referral of members to a mood and/or anxiety specialty treatment provider • Apply active listening, psychoeducation, and motivational interviewing skills to understand each member’s needs and provide support as they set goals • Utilize knowledge of Dialectical Behavioral Therapy (DBT) skills to coach members as part of their care journey • Employ motivational interviewing to assist members in engaging in care • Identify an appropriate level of care, facilitate referral, and coordinate care with a Spring Health provider • Provide consultation regarding mental health concerns and family/relationship issues, as appropriate to member needs • As necessary, provide in-the-moment support and safety planning for members with safety, risks or crisis • Utilize written communication skills to interact with members and providers through email • Document information, maintain accurate records, perform follow-ups, and track progress within several systems – including an EHR – within required timeframes • Identify and problem-solve issues that serve as a barrier or disruption to care • Interact with multi-disciplinary teams such as care support, provider operations, and benefits/billing to ensure a smooth member experience • Assisting other specialty care teams (SUD, Trauma, and Eating Disorders), as needed • Participate in staff meetings, case consultations, and trainings • Assist with onboarding and mentoring new team members • Follow Spring Health policies and maintain all confidentiality, compliance, and ethical standards

Job Requirements

  • Required mental health license: LCSW, LMFT, LPC
  • Minimum of 2+ years of clinical care experience working with chronic and severe mood and anxiety disorders
  • Experience with motivational interviewing, diagnostic assessment, and triage
  • Knowledge of current best practices and evidence based treatments for mood and anxiety disorders, with a strong preference for experience and training in Dialectical Behavioral Therapy (DBT)
  • Excellent listening, written and verbal communication skills
  • Acknowledge and embrace diversity and inclusion
  • Experience working with diverse populations
  • Motivated to go above and beyond for members to provide a high-touch experience
  • Tech-savvy: must be comfortable using various computer platforms and learning new systems, and efficient in tech-related tasks
  • Able to multi-task and context switch!

Role Description Trillium Health Resources has a career opening for a 1915(i) Triage Coordinator to join our team! The 1915(i) Triage Coordinator plays a vital role in the coordination of services for Trillium Health Resources members enrolled or referred to the 1915(i) program and is responsible for members identified through the 1915(i) Reassessment Dashboard and various internal and external referral sources. The 1915(i) Triage Coordinator is responsible for completing the required 1915(i) Assessments (initial, annual, and updates), CMCA (initial or annual), and CP/ISPs (initial). Based on the member’s diagnosis, eligibility, and service needs, the 1915(i) Triage Coordinator identifies and connects the member to the most appropriate internal team to ensure seamless transition and support. On a typical day, you might: - Provide education and support to members in learning about and exercising their rights and responsibilities within the 1915(i) framework. - Ensure compliance with best practices, program requirements, and Trillium Health Resources standards. - Coordinate and conduct comprehensive reviews of 1915(i) members by assessing available records, including medical and behavioral health documentation. - Complete record review screening processes, analyze assessments and medical documentation to assess eligibility and determine the member’s needs for I/DD, TBI, or MH/SUD services and supports. - Work with members and LRPs to ensure that assessments are updated as needed and appropriately to reflect the members’ service needs. - Coordinate ongoing referrals based on identified priority level, ensuring timely action is taken for each member. Qualifications - Must meet QP status: - Fully licensed by the NC governing board regulating Human Services professions; OR - Fully licensed RN who is licensed to practice in the state of NC by the NC Board of Nursing who also has four (4) years of full-time experience with the MH/DD/SA population served; OR - A graduate of a college or university with a Master’s degree in a human service field and has one year of full-time, pre or post-graduate degree accumulated MH/DD/SA experience with the MH/DD/SA, or a substance abuse professional who has one year of full-time, pre or postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling; OR - A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has two years of full-time, pre or post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; OR - A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has four years of full-time, pre or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling. - If serving members with LTSS needs, the Coordinator must meet the minimum QP requirements defined above (A-D) and shall additionally have at a minimum two (2) years of prior LTSS and/or HCBS coordination, care delivery monitoring and care management experience. This experience may be concurrent with the years of experience required to become a Qualified Professional. - A valid active driver’s license is required to be maintained for this position. - Must reside within Trillium’s Catchment of North Carolina. - Must be able to travel within catchment as required.
