Patient Care Coordinator

• Own patient navigation end-to-end: Schedule appointments, guide next steps, clarify risk levels, ensure follow-ups, and proactively re-engage patients who haven’t scheduled. • Be the voice of the client post-booking: Serve as the main point of contact via text/email/phone, delivering fast, empathetic support that builds trust and drives high satisfaction. • Outreach to patients: Perform outreach to patients who have been referred by their physicians or taken the risk assessment, converting them to scheduled appointments. • Drive order completion: Track imaging/testing orders from placement to completion so patients don’t fall through the cracks and follow-ups happen on time. • Build breast health expertise: Stay current on screening guidelines, risk assessment, and early detection to help patients understand what’s next and why it matters. • Coordinate across partners: Work closely with Breast Specialists, imaging centers, and payers to keep the care journey smooth and integrated. • Make the system better: Monitor the EMR and identify ways to streamline referrals, registration gaps, SBO/MSPQ issues, charge capture, and claim edits to reduce delays. • Perform insurance estimates: Utilize the EMR and insurance portals to determine patient responsibility before payments and communicate with patients. • Own pre-visit auth workflows: Secure payer approvals ahead of visits to minimize last-minute friction and keep care on track. • Be a team multiplier: Coordinate access needs and troubleshoot basic tech issues so the care team can stay focused on patients.

Job Requirements

  • 1+ years of experience in a customer experience, customer service, care coordination, or patient-facing role
  • Strong interest in women’s health; comfort supporting patients through sensitive, high-emotion moments with empathy and discretion
  • Tech-forward and eager to learn new tools/software quickly; comfortable working across multiple systems in a fast-paced environment
  • Proven ability to own an initiative end-to-end—from identifying a need to executing, iterating, and delivering results
  • Exceptional verbal and written communication skills; able to explain complex information clearly, kindly, and confidently
  • Natural problem-solver with a “figure it out” mindset and a bias toward action
  • Highly organized with impeccable attention to detail; able to manage multiple workstreams, deadlines, and follow-ups without things slipping
  • Comfortable with ambiguity and rapid change; thrives in a build-and-iterate startup environment
