Job Requirements
- A completed master’s or doctoral degree in social work, psychology, counseling, marriage and family therapy.
- An active clinical license (LCSW, LCSW-C, LMHC, LMFT, LPC, CP).
- Experience working with the geriatric population.
- Comfort with technology and experience providing teletherapy is a plus.

Our client is seeking a Cardiology Patient Navigator (Coordinator) to support a busy cardiology care team. Cardiology-specific experience is preferred but not required.
