Key Responsibilities - Accurately document patient history, physical exams, assessments, and treatment plans during live clinical visits - Transcribe verbal notes and update electronic health records (EHR) with precision - Review and edit medical documents for completeness and consistency - Assist with coding and billing documentation under provider guidance - Maintain confidentiality and comply with HIPAA regulations - Collaborate with physicians and clinical staff to streamline documentation processes - Track and manage patient follow-ups, referrals, and test results as needed Qualifications - High school diploma or equivalent (Bachelor’s degree in health sciences or related field preferred) - Prior experience in a medical or healthcare setting is a plus - Strong understanding of medical terminology and anatomy - Proficiency in EHR systems and typing (minimum 60 WPM recommended) - Excellent listening, communication, and multitasking skills - Ability to work in fast-paced environments with accuracy and discretion - Commitment to patient privacy and professional ethics Preferred Skills - Familiarity with ICD-10 and CPT coding - Bilingual fluency (e.g., Spanish-English) is a strong asset - Experience with remote scribing platforms or telehealth workflows
