The Treatment Nurse Navigator acts as a consistent point of contact, coordinating care, reducing barriers, and ensuring patients receive timely, evidence-based services across the continuum. The ideal candidate will have a strong clinical nursing background with experience in oncology or patient navigation, a commitment to patient-centered care, and the ability to collaborate effectively across multidisciplinary teams. Schedule: Shift: Monday - Friday, 8:00am - 5:00pm. This position does not currently include weekends or holidays. Orientation: Orientation may be 1 - 2 months onsite. After successfully completing the training period, this position may transition to a hybrid work arrangement. Onsite expectations may vary based on business needs. Responsibilities Care Coordination and Communication - Develop relationships with patients after the first appointment to establish a consistent navigation point of contact throughout the treatment phase - Assess patient, family, and caregiver needs post-first visit and throughout the continuum of care, including psychosocial, financial, and emotional changes - Perform distress screenings, document findings, and refer patients to appropriate disciplines - Track patient transitions between ambulatory and inpatient care settings and collaborate with care management teams - Coordinate consult appointments with support services such as social work, nutrition, genetics, and therapy within established service standards - Assist with treatment goals discussions, advance directives, palliative care, and end-of-life planning using an ethical framework - Apply knowledge of payer requirements and insurance processes to facilitate appropriate referrals and care decisions - Promote collaboration across care teams and external organizations to protect and advance patient interests - Perform other duties as assigned Education - Provide education on oncology diagnoses, treatment plans, support services, and local or national resources - Educate patients and caregivers on clinical trial opportunities in coordination with clinical and research teams Communication - Communicate concerns and provide follow-up to ensure timely coordination of appointments, tests, and procedures - Conduct outreach across UT MD Anderson to build and maintain relationships with physicians, advanced practice providers, nursing, case management, and social services teams Documentation - Develop concise patient summaries for care teams and document navigation recommendations using standardized care protocols - Utilize evidence-based navigation tools to assess patient needs including pain, fatigue, and performance status Professional Development and Performance - Participate in multidisciplinary tumor conferences and evaluate recommendations against NCCN or national guidelines - Engage in performance improvement activities to enhance patient, family, and caregiver experience - Complete required training, education, and annual core competency programs - Maintain knowledge of local and national patient resources - Adhere to productivity benchmarks, caseload expectations, and documentation standards supporting program performance Other duties as assigned. EDUCATION - Required: Bachelor's Degree Nursing. - Preferred: Master's Degree Nursing. WORK EXPERIENCE - Required: Five years Professional oncology nursing or oncology nurse navigation experience. - Required: May substitute required degree with an Associate's degree in Nursing and seven years of professional oncology nursing or oncology nurse navigation experience. LICENSES AND CERTIFICATIONS - Required: RN - Registered Nurse - State Licensure State of Texas Professional Nursing License (RN).
